Delivers care according to the Nursing Department Standard of Care oAccurately takes and records patients' B/P, TPR and weight. oNotifies RN of changes in vital signs. oAssists patient in maintaining nutritional and hydration needs, monitors and assists with elimination needs as appropriate for the patient's age, development and physical status. oAccurately measures and documents patient's input and output as needed. oProvides constant observation, when needed, as directed by the Registered Nurse oAssists patients in following through with ADLs (i.e. bathing, laundry, organizing room), as appropriate for the patient's age, development, and clinical needs. Communicates accurately and in a timely manner via verbal reports to the charge nurse and other members of the clinical team, written reports, and the electronic medical record as appropriate and required by unit procedure. oPerforms appropriate daily documentation. Enters progress notes, group notes, vital signs, patient belonging information, and other data as indicated or assigned, into the medical record. oCollaborates with the primary nurse/charge nurse regarding implementation of a patient's daily plan. Participates as a member of an interdisciplinary team. oShares observations of patient behavior in clinical meetings. oAttends nursing staff meetings and team meetings when scheduled. oParticipates in unit based Quality Improvement Plans. oAttends meetings and participates in committees as required. Participates in unit monitoring and unit safety. oFollows unit safety maintenance and checks procedures at all times, and demonstrates accurate and timely documentation of safety checks surveillance, and close observation. oKnowledgeable about hospital fire and safety procedures and completes annual reviews. oMaintains current BLS certification for Healthcare Providers. oHas completed training, and is able to act appropriately in a physical hold/restraint procedure. Familiar with DMH guidelines and regulations on restraint and seclusion of children and adolescents. Understands and implements the Physical Hold Restraint Policy when indicated. oDemonstrates an understanding of the principles of caring for patients across the lifespan and trauma informed care. oDemonstrates responsibility for maintaining unit supplies and electronics, including checklists and inventory lists. oParticipates in maintaining the cleanliness of the unit on each shift. oAssists in emergency situations and drills as appropriate (fire, disaster first responder). Participates in facilitating a therapeutic milieu utilizing communication and interactional skills. oParticipates as an integral member of the unit-based group treatment program team. oLeads and co-leads a variety or groups as assigned by the nurse manager/charge nurse. o(May include recreation, education, psychoeducation, and activity groups.) oPlans unit activities with the charge nurse and accompanies patients on walks and outings as assigned. oReviews and follows the nursing care plan for each patient assigned to him/her. oDemonstrates initiative in establishing relationships with patients through walks, group activities, individual meetings and activities, etc, oDemonstrates availability to patients and active engagement in the development and maintenance of the therapeutic milieu. oDemonstrates appropriate use of collaboration, negotiation, limit setting and other strategies to promote a positive learning experience for the child and adolescent patient. oWorks with individual patients to plan and utilize milieu therapeutically. oIncorporates the principles of trauma informed care and strength-based approaches to formulations and care. oLearns from work experience, colleagues, and supervision, and applies new knowledge to further develop skills. About the role Job Summary: The Clinician I will assist the Director of Outpatient Services ensuring the provision of quality care in an outpatient mental health clinic serving individuals with co-occurring disorders. S/he will provide weekly supervision to Master Level clinicians, interns, or other members of the interdisciplinary team to ensure clients receive comprehensive assessments and individualized service plans; and provide group treatment services using evidence-based practices, as needed. They will also ensure service utilization benchmarks and reporting requirements for funders are met in a timely manner. As part of the Casa Esperanza, Inc. team, the Clinician I will provide services in a fully integrated, holistic, and collaborative manner using a team-treatment approach, to ensure the coordination of all client services, both across Casa programs as well as with external providers. The Clinician I will support with creating, implementing and delivery trauma informed care across Casa programs. This integrated model aims to facilitate clear communication amongst all providers and eliminate duplication of efforts or conflicting approaches to care; detail necessarily follow up and assignments; and ensure-wrap around support services are in place to maximize treatment effectiveness and clinical outcomes. What you'll do Clinical Services Maintain an up-to-date waitlist and prioritize the assessment and appropriate placement of all high-risk individuals. Monitor case assignments and ensure appropriate caseload mix. Facilitate group interventions using evidence-based practices, as needed. Provide support to clients through individual therapy Utilizes case conferencing and the program team to coordinate and ensure client treatment plans are effective and appropriate. Support with clinical group supervision to ensure effective communication among team members, manage productivity, and quality assurance. Provide ongoing support as need for facilitation of the multidisciplinary team meeting and office-based addiction treatment meetings. Educate clients and their families about psychiatric and substance use disorder diagnoses; recovery and wellness support services; medication including MAT for substance use disorders, psychotropic medications, and medications for other physical conditions; and ensure engagement with medication monitoring services and regular health maintenance Build collaborative relationships within the community that support a strong recovery system of care. Work with Director of Outpatient Services to ensure that all clinical staff receive training and regular performance evaluations that support the delivery of safe, effective, evidence-based, and culturally competent care. Work with Director of Outpatient Services team to plan, implement, and maintain urgent care model. Provide back up on-call coverage, as needed. Complete all clinical documentation in the agency electronic record. Provide SOAP coverage, as needed Other duties as assigned Administrative Work closely with the Director of Outpatient Services to ensure Casa Esperanza Outpatient facilities meet all licensing, regulatory, accreditation, credentialing and funder standards and requirements. Maintains client and program records in accordance with agency, regulatory, and funder requirements and timelines Work with Intake and admission Manager and Director of Outpatient Services to maintain efficient workflow to meet program utilization goals and objectives and ensure timely billing. Work with Director of Outpatient Services to respond to internal and external data collection and reporting requirements. Complete intern progress recordings, evaluations, and all required school documents Participate in any required site visits and ensure compliance with any funder requirements. Attend all relevant case conferences and staff meetings, including MDT. Support with facilitation of MDT, OBAT, and FUOP staff meetings. Provide weekly supervision to direct reports. Attend weekly supervision Attends all trainings and professional development opportunities as required. On call on a rotation schedule Ensure that all program components and evidence-based practices are delivered to the target population and address any barriers to implementation. Collaborate with Director of recovery Support Services to oversee data collection and outcomes reporting, including progress on goals. Support with creation, implementation, and delivery of trauma informed services in our integrated care model Support with SOAP enrollment and tracking of completion Other duties as required Qualifications Master's degree in social work, psychology, counseling, or related field required Licensed Independent Clinical Social Worker (LICSW) or Licensed Mental Health Counselor (LMHC) Bilingual Spanish/English required Two years' experience working individuals living with co-occurring addiction, mental illness and/or other trauma related disorders Supervisory experience in a clinical setting and program management experience Knowledge of appropriate community resources Experience with homelessness, domestic violence, chronic disease, and incarceration Experience in crisis management and intervention Experience providing individual, family and group interventions to target population. Schedule: Full-time, at least 40 hours. Monday-Friday. Pay rate: $65,000-$67,200 (Masters), $69,400-$73,800 (LADC-1), $77,100-$88,100 (LMHC/LICSW).
