Position Summary:
The Social Worker is responsible to facilitate care along a continuum through effective resource coordination to help patients achieve optimal health, access to care, and appropriate utilization of resources, balanced with the patient's resources and right to self-determination. The individual in this position has overall responsibility for to assess the patient for transition needs including identifying and assessing patients at risk for readmission. Conducts complex psycho-social assessment and intervention to promote timely throughput, safe discharge, and prevent avoidable readmissions. This position integrates national standards for case management scope of services including: Transition Management promoting appropriate length of stay, readmission prevention, and patient satisfaction; Care Coordination by demonstrating throughput efficiency while assuring care is the right sequence and at appropriate level of care. Compliance with state and federal regulatory requirements, TJC accreditation standards, and Tenet policy; Education provided to physicians, patients, families, and caregivers; and Leads a population of patients by service line and/or leads the team by being a resource to Tenet performance standards.
Responsibilities
This individual's responsibility will include the following activities:
a) complex psycho-social transition planning assessment and reassessment and intervention, b) assistance with adoptions, abuse and neglect cases, including assessment, intervention and referral as appropriate to local, state and /or federal agencies, c) care coordination, d) implementation or oversight of implementation of the transition plan, e) leading and/or facilitating multi-disciplinary patient care conferences including Complex Case Review, f) making appropriate referrals to other departments, g ) communicating with patients and families about the plan of care, h) collaborating with physicians, office staff, and ancillary departments, I) assuring patient education is completed to support post-acute needs, j) timely complete and concise documentation in Case Management system, k ) maintenance of accurate patient demographic and insurance information, l) precepts new staff members and acts as a resource to all staff, m) facilitates TEMPO as needed, n) participates in department quality improvement initiatives, and o) other duties as assigned.

Complete clinical documentation for each client including, but not limited to: intake assessments, progress notes, treatment plans, termination summaries, and other documentation in a timely fashion.

About the Position
We are looking for a Residential Therapist to join our team and provide on-site clinical services for our Clear Vue RTH and Pine Ridge RTF in Springfield, Oregon. This position provides clinical services and supervision for individuals affected by a severe and persistent mental illness in a residential treatment setting. This position requires the ability to assist with training and clinical oversight of the Residential Associates who provide the 24-hour residential support services to the residents. The Residential Therapist will report to the Clinical Manager for day-to-day supervision and work assignments, and be supervised by the ColumbiaCare Clinical Director for clinical standards and quality improvement.
The Residential Therapist position requires the ability to perform a Comprehensive Mental Health Assessment, Assessment Updates and Treatment Plan. The Residential Therapist will also provide clinical oversight and consultation to facility staff and liaison with other community providers. This position will need to be accessible by phone for emergency purposes. In addition, you will receive crisis de-escalation training, which requires performing various physical, hands-on maneuvers and techniques.
Work Schedule: Monday through Friday 8:00am - 5:00pm (Full Time, Day).

Clear Vue (set. 2015) is a 5-bed, Residential Treatment Home that serves individuals who have a mental illness, and are under the jurisdiction of the Psychiatric Security Review Board (PSRB).
Pine Ridge RTF is a 7-bed Residential Treatment Facility (RTF) located in Eugene, Oregon. Pine Ridge RTF serves adults who are living with a Serious Mental Illness and have been deemed unable to Aid & Assist in their own defense.

About the role
Job Summary: The Clinician I will assist the Director of Outpatient Services ensuring the provision of quality care in an outpatient mental health clinic serving individuals with co-occurring disorders. S/he will provide weekly supervision to Master Level clinicians, interns, or other members of the interdisciplinary team to ensure clients receive comprehensive assessments and individualized service plans; and provide group treatment services using evidence-based practices, as needed. They will also ensure service utilization benchmarks and reporting requirements for funders are met in a timely manner. As part of the Casa Esperanza, Inc. team, the Clinician I will provide services in a fully integrated, holistic, and collaborative manner using a team-treatment approach, to ensure the coordination of all client services, both across Casa programs as well as with external providers.
