GENERAL RESPONSIBILITIES:
Facilitate the person-centered planning process
Provide Case Management Services to individuals on assigned case load through:
Transportation as required by treatment plan
Development of Natural Supports including family, friends, religious organizations/leaders, neighbors, teachers, employers, and all others involved and interested in supporting the consumer
Internal and external case collaboration and coordination, including gathering and exchanging relevant information
Participation in clinical case conferences, staff development (in-services, conferences, professional organizations) and agency committees as assigned
Support individuals on assigned caseload through advocacy, monitoring, referring, and linking with other providers

Collaborate with prescribing providers to align services with clients' needs and treatment goals
Ensure service plans comply with agency policies, Medicaid guidelines, and other regulatory requirements
Facilitate coordination between psychiatric and medical services to address both mental and physical health needs, when necessary
Assist clients in identifying and establishing a primary care provider (PCP) for non-psychiatric medical needs
Educate clients on the importance of integrated healthcare, including medication adherence and preventative care
Advocate for client needs within the healthcare system, ensuring they receive appropriate and timely services
Document all case management activities in accordance with agency policies and confidentiality guidelines
Maintain up-to-date clinical records and service documentation in the electronic health record (EHR) system
Ensure compliance with HIPAA, Medicaid, and agency policies regarding client confidentiality and service provision
Track client engagement and follow up on missed appointments, care coordination needs, or changes in mental health status
Perform other duties as assigned
