Job Summary
We're seeking a professional, empathetic, and respectful Integrated Health Therapist (IHT) who is able to provide clinical diagnoses for treatments and create individual treatment plans for patients experiencing issues related to mental and behavioral health, chemical health, psychosocial aspects of health and disease, and lifestyle management. Responsibilities
Comfortable providing coverage across all populations and patients with co-morbid medical conditions
Provides diagnostic assessments, onsite crisis assessments and interventions, determining need for ongoing care according to level of clinical ability, medical and psychosocial complexity
Competent in multiple counseling modalities, making appropriate referrals, and providing educational services
Serves as the primary team consultant to promote understanding of the relationship between health and psychological/behavioral factors
Works closely with other PCMH or PCC team members to address psychosocial and cultural elements of health and disease pertinent to the patient's ability to manage their acute and chronic conditions
Documents in a responsible, accountable and ethical manner to promote safe, reliable patient centered care
Qualifications
Master's degree in Counseling, Social Work, Psychology, or related Behavioral Health Sciences required; graduation from an accredited school preferred.
License must be in good standing in state(s) of practice.
In Iowa, must have one of the following:

Graduate of respiratory therapy program required.
Credentialed by the NBRC at the Certified Respiratory Therapist or Registered Respiratory Therapist level required.
Current Respiratory Care Practitioner licensure in the state of Iowa required.
CAPRS and N95 fit testing validation within 30 days of hire required.
Must have demonstrated competence in Respiratory Care.
Must provide proof of Basic Life Support (BLS Healthcare Provider, American Heart Association; or American Red Cross, Professional Rescuer-LIVE class only) via current card prior to employment or within 60 days of hire and maintains current status.

Responsibilities:
Works as part of a multidisciplinary team across Healthy Start/Empowerment, EveryStep and the service area.
Assess for client's needs by performing psychosocial assessments and diagnostic practice using the current edition of the Diagnostic and Statistical Manual of Mental Disorders (DSM-5-TR).
Develops personalized care and treatment plans to address physical, emotional and mental wellbeing, including the establishment of goals, psychosocial therapy using evidence-based therapeutic modalities, differential treatment planning, and crisis planning.
Provides a safe space and environment for culturally responsive, family-centered mental health counseling for program participants and their families. This includes work in program participants' homes.

Qualifications:

A Brief Overview

Duties and Responsibilities:
The Drug Addiction Counselor reports to the Health and Wellness Manager (RN) and a salaried, exempt position.
The Drug Addiction Counselor is responsible for the designs, develops, and implements all programs related to substance use and abuse prevention and intervention in conjunction with the Mental Health Consultant and Health Services staff, center physician and Center Director.
The Drug Addiction Counselor coordinates all programs designed to reduce and eliminate substance use and abuse and in compliance with the Department of Labor/Job Corps guidelines.
Conducts in-service training sessions with both staff and students in all areas related to substance use and abuse.
Submits recommendations to the Center Director for medical terminations related to substance misuse and abuse in conjunction with the Mental Health Consultant, center physician, Health and Wellness Manager (RN), etc.
Monitors behavior and progress of students involved in the Alcohol and Other Drugs of Abuse Prevention/Intervention Program.
Maintains confidential reports and records on all referrals and cases.
Qualifications:
Active, unrestricted substance abuse license or certification that meets minimum state licensing or certification requirements to practice in the state where Center is located.
Prefer One year of work-related experience.
Prefer experience working with youth.
Prefer Master's degree and previous experience with disadvantaged youth, particularly in a residential setting.
Valid driver's license with acceptable driving record preferred.
Required to pass a drug screen in compliance with our Substance Abuse and Drug-Free Workplace policy.
Additional Requirements: Broad-based knowledge of counseling techniques. Knowledgeable in the area of alcohol and other drugs of abuse intervention programs. A Brief Overview
Responsible for the daily coordination and delivery of physical therapy services implementing specific treatment program for individual patients and patient groups according to the principles and practices of physical therapy and established objectives of the department.
What you will do
Evaluates assigned patients, gathering data from a variety of sources such as patient records, interviews, observation and team members.
Develops and implements individualized treatment programs for the patients needing services. Reviews plan of care with patient. Instructs patient, family, caregivers in home program or exercise as appropriate.
Uses selected activities and/or tasks to promote and maintain health, and to achieve treatment goals, establish and modify intervention and strategies for patients' treatment program based on ongoing evaluation.
Provides direct therapy services to patients using various functional settings and strategies, plans therapy within the patient's individual physical, emotional and intellectual capabilities as indicated and coordinated with the rehabilitation, medical, and nursing care plans.
Prepares documentation as required by the profession and the department, such as evaluation results, individualized treatment plans, progress notes, and other such reports.
Communicates effectively with the patient, family, physician, and other disciplinary team members regarding the patient's status, progress and needs. Attends care conferences when applicable.
Establishes rapport and work cooperatively with interdisciplinary team members (including family/physician) throughout the patient's intervention program.
Enhances professional growth and development through participation in educational programs, current literature, in-service meetings, workshops, etc.
Oversees physical therapy assistants, techs, and students in an efficient and effective manner that ensures safe patient care and smooth departmental operation.
