Maintain relationships with all existing referral sources.
Actively prospect for new referral sources based on the Agency's scope of service.
Work closely with Marketing, Agency Administrator and Clinical Management to ensure that the demand for clinical care is aligned with our capacity and at no time compromises the Agency's ability to provide the best quality care for our patients.

This individual's responsibility will include the following activities: a) complex psycho-social transition planning assessment and reassessment and intervention, b) assistance with adoptions, abuse and neglect cases, including assessment, intervention and referral as appropriate to local, state and /or federal agencies, c) care coordination, d) implementation or oversight of implementation of the transition plan, e) leading and/or facilitating multi-disciplinary patient care conferences including Complex Case Review, f) making appropriate referrals to other departments, g ) communicating with patients and families about the plan of care, h) collaborating with physicians, office staff and ancillary departments, i) assuring patient education is completed to support post-acute needs , j) timely complete and concise documentation in Case Management system, k ) maintenance of accurate patient demographic and insurance information, l) and other duties as assigned.

· Completes Final Discharge Disposition Form Assessment for Medicare patients per Tenet policy

Care Coordination
· Screens patients for factors that may affect the progression of care and intervenes as needed to promote timely and appropriate throughput
· Conducts assessments and stratifies patients at risk for readmission or in need of Case Management services
· Assists with adoption/abuse/neglect cases and reporting of appropriate cases to local, state and/or federal agencies
· Ensures the plan of care is consistent with patient choice and available resources
· Ensures patient needs are communicated and that the healthcare team is mutually accountable to achieve the patient plan of care
·Effectively collaborates with physicians, nurses, ancillary staff, payors, patients and families to achieve optimal outcomes(40% daily, essential).
Education

Qualifications:

The Children's COP SUD Therapist is responsible for maintaining a caseload consisting of children, adolescents and families, with a primary substance abuse and/or mental health diagnosis. We have a market-based compensation structure. The estimated salary for this position will vary based on a candidate's transferable work experience and qualifications. Salary ranges start at $52,026.00 per year. The range provided is just one component of total compensation including a comprehensive benefit package of affordable medical plans, employer paid dental, vision and long-term disability, 401(K), flexible spending accounts for medical and dependent, paid time off, 11 paid holidays.
The Adult Behavioral Health SUD Therapist is responsible for maintaining a caseload consisting of clients, adolescents and adults, with a primary substance abuse and/or mental health diagnosis. The therapist is required to conduct intake assessments, to maintain the case record in a timely fashion, to complete authorizations with various funding sources in a timely manner as required, and to conduct appropriate treatment services based on medical necessity. Regular working hours for staff are Monday through Thursday 8:30 am to 7 pm and Friday 8:30 am to 5 pm and Saturday 9 am to 1 pm. Evening hours will be required and determined based on program needs.
Requirements:
Must have a minimum of a Master's degree in Social Work, Psychology or Counseling and possess the corresponding State of Michigan licensure Candidates without a state license can be considered but must obtain state licensure (LLMSW, LLPC, TLLP, LMSW, LPC or LLP) within a 60-day training period beginning on the date of hire. This period may be extended by up to an additional 60 days of training at the sole discretion of the employer. Failure to obtain licensure within the training period will result in the employee being unqualified to perform work functions. Beyond the training period, all state licensure must continually remain active, in effect, and free from disciplinary action.
Candidates must obtain the Qualified Mental Health Professional credential within 120 days of hire. Must possess a CAADC credential, or be on an active, approved registered MCBAP certification Development Plan within six weeks of hire.
Must have one year of experience (may include time spent in fieldwork or internship) treating or working with people who have been diagnosed with mental illness and substance use disorders.
Must be able to work independently.
Must have excellent written, verbal, and interpersonal communication skills.
Must have excellent organizational and attention to detail skills in order to track and prepare data accurately and timely.
Must have the ability interpret a variety of instructions furnished in written, oral, or schedule form.

Summary Description

· Screens patients for factors that may affect the progression of care and intervenes as needed to promote timely and appropriate throughput
