Deploys evidence-based interventions such as motivational interviewing, problem focused therapy, mindfulness, cognitive behavioral therapy, and acceptance and commitment therapy. Assists patients in developing skills to facilitate disease self-management. Develops and maintains a collaborative relationships with the primary care physicians, ambulatory practice team, other behavioral health team members, patients and families to include, providing behavioral health education to staff, patients and families in individual and group settings. Works collaboratively with community mental health providers and community support services to develop, provide and support a continuum of care for patients. Works collaboratively with the primary care team to provide referral, case management and follow up services for patients with behavioral health needs to include, engaging patients, initiating referrals, addressing barriers to care and follow-up. Employee should be familiar with community resources to address patient psychosocial and behavioral health needs. Provides accurate and timely documentation in Epic, Agility or other authorized electronic health records to support the identification and treatment of patient's psychosocial and/or behavioral health needs. Provides assistance in capturing program evaluation and fidelity measures as directed. Works collaboratively to assist in monitoring the behavioral health program and identifying areas for improvement. Participates in meetings and quality improvement activities, continuing education and serve as a member of committees, as requested.
