Conduct comprehensive medication reviews to identify, resolve, and prevent medication-related problems. Provide patient counseling and education on medication use, adherence, potential side effects, and health maintenance. Develop personalized medication action plans and ensure proper medication management and adherence. Collaborate with healthcare providers to optimize medication therapy and enhance patient outcomes. Manage inventory and procurement of medications and supplies, ensuring proper storage and handling. Assist patients with insurance claims, prior authorization processes, and finding cost-effective medication alternatives. Participate in the development and implementation of community health initiatives and wellness programs. Preferred Qualifications: Bachelor's of Pharmacy or PharmD degree from an accredited pharmacy program. Experience in community or independent pharmacy. State pharmacy license in good standing. Proficiency in utilizing pharmacy management software and electronic health record (EHR) systems. Strong analytical and problem-solving skills. Excellent communication and collaboration abilities.
