THE OPPORTUNITY Premier Medical Associates is seeking a team-oriented Clinical pharmacist to assist our practice. in optimizing utilization. ABOUT THE ROLE The Clinical Pharmacist will be responsible for supporting NeueHealth's pharmacy operations. The Clinical Pharmacist will be responsible for providing support to clinical programs, managing oversight of the drug pipelines (generic and new brands), and providing support to each market to increase drug affordability, improve quality measures and positively impact the member experience. Daily responsibilities include and are not limited to: Monitoring drug utilization in conjunction with Medical Analysts and assisting the leadership team in understanding quality and cost control issues related to pharmacy Reviewing patient medication profiles and applying evidence-based medicine and guidelines when creating recommendations to physicians for optimizing medication regimens Analyzing and reviewing medication lists on inpatient admissions and the CCM high-risk patients for suboptimal medication regimens and potential safety problems including drug interactions Working with health insurance pharmacy directors and their PBMs to manage formulary changes and providing updates to the clinical leadership Serving as a formulary, pharmacy benefits and drug information resource Assisting Health Service, Network, Quality and HEDIS/STARs departments Assisting the Chief Medical Officer and Pharmacy Director in planning and implementing new and revised clinical programs EDUCATION, TRAINING, AND PROFESSIONAL EXPERIENCE Required Qualifications Doctor of Pharmacy Degree (PharmD) is required Ten (10) years' experience in the pharmacy industry Preferred Qualifications PGY1 Residency (Pharmacy Practice, Ambulatory Care or Community) or 5 years' experience in in-patient hospital, ambulatory or transitional care environment Board Certification (BCPS, BCACP, Geriatrics) Spanish speaking Medication Reconciliation OR Medication Therapy Management (MTM) experience Knowledge and experience of Medicare Part D, Individual & Family Plans (IFP), Commercial and Medicaid pharmacy business is preferred. ESSENTIAL JOB DUTIES/RESPONSIBILITIES: * Acts as a national pharmacy consultant for our physicians regarding patient case inquiries, drug interactions, therapy suggestions, and more. Our aim is to choose the most cost-effective medications to achieve the best outcomes for our patients. * Analyzes data and trends related to internal prescriptions and medication orders for appropriateness, focusing on safety and efficacy, while assisting in the creation and management of the formulary. * Reviews and verifies prescriptions and medication orders for appropriateness, including safety and efficacy. * Conducts prospective drug utilization reviews, evaluating proper dosage, usage directions, drug interactions, duplicate therapies, and other regimen-related factors, making case-by-case and population health interventions as necessary. Provides timely access to reliable drug information from available sources. * Oversees pharmacotherapeutics services in accordance with institutional practice standards and or protocols collaborating with specialists to develop clinical guidelines for our clinicians in the centers. * Creates and implements programs, SOPs, and guidelines that enhance practice and patient outcomes. * Provides efficient cost-effective patient centered care that minimizes hospitalization expenses * Provides daily verbal and written consultations about medication therapy. * Engages in the implementation and maintenance of guidelines and order sets alongside the department leader and other specialists. * Contributes to the education and mentorship of pharmacy residents and students by creating standard operating procedures for the program and overseeing their execution. * Works in collaboration with Part D Clinical Pharmacist to deliver presentations, publications, and other informative activities related to specialty practice, as well as Part D education for HEDIS measures. * Acts as the pharmacy department's representative on pertinent committees and teams, taking on the role of chair for the P&T meeting. In this position, you will be responsible for creating the slide deck, gathering information, and conducting research to present recommendations from the pharmacy department. Provide input and guidance for management of medication formulary alongside the Pharmacy COE leader. * Follows established department policies, procedures, objectives, quality assurance programs, safety, environmental and infection control products. * Supports and facilitates the Quality Improvement process through the continuous development and monitoring of performance improvement indicators. * Fosters growth and effectiveness of Pharmacy work teams to meet quality and clinical care standards/ * Assists Pharmacy leadership in ensuring the effective and efficient use of resources while meeting budget and value proposition targets. * Performs other duties as assigned and modified at manager's discretion.
