Ensure accuracy of all pharmaceutical products prepared or processed by technical staff. Work closely with clinical staff in developing patient wellness programs such as smoking cessation programs and asthma classes. Provide medication information to the clinical staff as required, including in-service training on new medications on the market; appropriate use of medications; comparison of current medication therapies; and other topics as deemed appropriate by the department head. Provide clinical direction of corpsmen and pharmacy technicians and provide input into job performance evaluations. Maintain inventory control of all pharmaceuticals in the pharmacy, especially scheduled pharmaceuticals, and ensure pharmacy security. Maintain appropriate records and security of all scheduled controlled substances according to applicable regulations. Utilize pharmacy computer and robotic systems in accordance with MTF protocols. Participate in a multidisciplinary team with other health care providers providing consultative services pursuant to optimal health care delivery to patients suffering from pain. Work with physicians in a multidisciplinary team to provide drug therapy recommendations, perform polypharmacy management, and provide patient education regarding changes in drug therapy. Conduct a deliberate medication review and reconciliation for chronic pain patients and develop strategies for patients with histories of non-compliance, abuse and or diversion. Conduct a clinical interview with patient, family, and/or caregiver(s) to identify information necessary to design a pharmacotherapeutic treatment plan. Order and evaluate appropriate laboratory tests to assess clinical response, development of adverse drug reactions, therapeutic drug levels, and complete all appropriate clinical reminders. Document and communicate all information as appropriate; pain patient agreements, pharmacotherapeutic treatment findings and recommendations. Educate patients and their families/caretaker(s) on medications and non-drug therapy and provide appropriate written drug information: counsel on self-monitoring techniques and lifestyle changes to promote wellness; emphasize medication adherence and provide compliance aids, such as medication charts and pill boxes. Extend existing prescriptions (psychotropic and non-psychotropic medications) as appropriate and ensure the patient receives follow-up appointment(s) with the appropriate individual(s). Order consults and referrals as appropriate. Participate in an on-site designated Extension for Community Healthcare Outcomes (ECHO) tele-mentoring training program. Participate in tele-mentoring, tele-medicine, and tele-consults. Minimum Qualifications: Doctor of Pharmacy (Pharm.D.). Graduate from a college or university accredited by the Accreditation Council on Pharmaceutical Education (ACPE). Completion of a clinical pharmacy residency or fellowship accredited by the American Society of Health System Pharmacists or American College of Clinical Pharmacy or documentation of appropriate education, training, and/or CME in the practice of clinical pharmacy. Possess a graduate certificate from an accredited pharmacy residency program OR Possess a minimum of twelve (12) months of experience within the preceding eighteen (18) months as a Clinical Pharmacist performing direct patient care clinical activities in an ambulatory care pharmacy setting OR Have held clinical privileges as a Clinical Pharmacist within the preceding thirty-six (36) months in a military treatment facility. Current, full, active, and unrestricted license to practice as a pharmacist. Must be a US Citizen.
