Operations
* Counsels' patients and answers their questions regarding usage of medicine, side effects, interactions, contraindications, patient information privacy, generics, less expensive medicines, over-the-counter products, and refers to medical provider as needed to ensure medication is taken correctly, health needs addressed, and satisfaction with service.
* Performs pharmacist tasks including compounding, drug therapy reviews, verification, and medication management.
* Reviews, interprets, and accurately dispenses prescribed medications when

* Ensures the pharmacy operates in accordance with regulations, company policies and standards. * Maintains current knowledge of information technology associated with pharmacy systems including workflow, prescription fulfillment, billing, clinical documentation, training, inventory management, and point of sale registers to provide support to patients as well as pharmacy staff. Seeks new and better ways to further promote productivity.
* Ensures the accurate processing of insurance claims to resolve patient issues and prevent payment rejections. Follows up with insurance companies and medical providers and participates in 3rd-party audit.
* Follow-up with medical providers' offices to clarify prescribed medications, dosages, refills, interactions, and allergies to suggest alternative medications, and answer medical provider questions.
* Performs clinical and wellness services such as immunizations, diagnostic testing, disease state management and other healthcare services.
* Assists and supports the pharmacy manager in analyzing performance data including pharmacy financial, customer service, and implementation of procedures for pharmacy asset protection and inventory management.

Basic Qualifications

SUMMARY: The Pharmacist will provide accurate medication packaging and distribution services to Adult/Youth in long term care facilities. Communicating with a multidisciplinary health care team, the Pharmacist will ensure that each Adult/Youth's prescription therapy is safe and appropriate. DUTIES AND RESPONSIBILITIES:
Verify prescriptions entered in pharmacy system including correct patient, drug and dose.
Check for drug-drug/drug-disease interactions to prevent adverse drug reactions.
Communicate with prescribers to clarify unclear or potentially harmful orders.
Council patients and/or caregivers on any adverse drug reactions, interactions with other medications or food and alcohol.
Advise patients and/or caregivers about general health topics - diet, exercise, stress management, etc.
Interact and engage with insurance companies and prescribers to ensure payment and resolve coverage denials.
Complete a medication reconciliation when there are transitions in care to resolve medication discrepancies and ensure patients are getting the correct medication regimen.
Perform Medication Therapy Management services including comprehensive medication reviews, interventions and education. Bill for these services.
Manage staff overseeing and mentoring pharmacy technicians, student interns and residents.
Maintain accurate inventory of medications.

SUPERVISORY RESPONSIBILITIES:

QUALIFICATIONS:
Be a Registered Pharmacist in good standing in the State of Wisconsin;
Possess a Bachelor's degree in Pharmacy. A Doctor of Pharmacy is preferred;
Meet all licensing requirements set forth by the respective state of operation;
Possess a valid, regular (non-probationary) state driver's license if the individual will be driving either their personal vehicle for on-the-job purposes or any agency vehicles;

Meet the background check requirements for each state's regulations;
Successfully pass an initial medical examination and pass a drug screening, which provides evidence that they are free from communicable diseases, including active tuberculosis or conditions which prohibit them from reasonably performing the assigned duties, except as provided for by the ADA. Staff will be reexamined on a regular basis.
Have a minimum of two (2) years of supervisory experience in a Long Term Care pharmacy environment (preferred);
Have WPQC (Wisconsin Pharmacy Quality Collaborative) certification, or obtain certification within 180 days of assuming Pharmacist role.
