Job Functions: This is a customer-facing position with general customer service duties including, but not limited to, providing assistance, resolving issues, and ensuring a positive customer experience. Assisting store, restaurant, and truck care departments with general cleaning, maintenance, stocking, and customer service needs. Will serve as a liaison and discharge planner for Southwest Virginia Mental Health Institute (SWVMHI) and all state psychiatric facilities, will be expected to be onsite at SWVMHI as needed, will participate in treatment team meetings at the hospital, will develop and implement discharge plans, will maintain a caseload of individuals in state psychiatric facilities, will coordinate services and follow-up care with the outpatient programs of CMCSB, will attend regularly scheduled staff meets in order to coordinate discharge planning and successful community reintegration for individuals in state psychiatric facilities, will have a broad knowledge of the CMCSB service delivery system, will attend regularly scheduled regional meetings, will work closely with the DAP coordinator at CMCSB to ensure resources are secured at the time of discharge, will adhere to required documentation standards, will provide transportation to individuals at the time of discharge, will work effectively with DD services to ensure adequate discharge planning following state hospital psychiatric admissions.

Basic Qualifications High school diploma or GED. One year work experience, in a manufacturing environment or other applicable job-related experience Department of Defense Secret security clearance is preferred at time of hire.
