* Check to make sure that each student has his/her hearing aid, cochlear implant, and/or glasses daily;
* Attend to students' personal hygiene needs and the development of appropriate daily living skills;
* Assist students during mealtimes by providing supervision at each dining room entrance, checking assigned students at mealtime for appropriate behaviors, encouraging students to eat balanced meals, conversing with students during mealtimes, and helping to create a relaxed, family-type atmosphere; and
* Prevent child abuse and maltreatment by attending all staff training, carrying out responsibilities as a mandated reporter of child abuse and maltreatment, and under direction of the supervisor, use approved techniques to manage students, and report to supervisors, as required, regarding occurrences giving rise to concerns involving children in care.
Minimum Qualifications MINIMUM QUALIFICATIONS: Candidates must be able to read, write and speak English, follow written and spoken instructions, and be in good health and good physical condition.

Part-time Description
Under the supervision of the Director of Programs, the Community Habilitation Specialist (CHS) provides support in a home or community setting that promotes success at the highest level of independence possible. In addition to program-specific functions, this will include but not be limited to: teaching and reinforcing a wide range of daily living skills, promoting independence, prevent and respond to crisis situations appropriately, encouraging self-advocacy and facilitating the development of natural supports. All services will be provided in collaboration with program participants and their service plan, utilizing a person-centered approach.
Requirements
Responsibilities will include but are not limited to the following:
Establish respect and develop relationships with individuals using a person-centered approach. The Community Habilitation Specialist will become familiar with an individual by getting to know them through assessment, observation and discovery.
Provide teaching and assistance as needed to complete a daily living task or skill such as; cleaning, laundry, cooking, personal care, banking, etc.
Promote advocacy with the individual by seeking out services, promote choice, and assist in their understanding of rights and responsibilities.
Assist the individual with communication with their support team including their family, natural supports and community resources.
Respond to crisis situations by using good judgment through prevention, intervention, resolution, and reporting to ensure the safety and wellbeing of the individual.
Maintain necessary records and documentation for each consumer served.
Maintain confidentiality of all work/client related information.
Act in a manner consistent with Central New York Quest's Mission.
Meet mandatory training requirements (annual, quarterly & as needed trainings)
Meet with the Director of Programs or other appointed person for supervision at least once per month.
Must have reliable means of transportation as services take place in the clients home and community.
Other duties as assigned by the Director of Programs.
Requirements:
Valid Driver's License & willing to transport clients in own vehicle
2 years experience working with individuals with developmental disabilities (preferred)
High School Diploma
This position is a part time position
