Per Diem Chaplin Day Kimball Health The Chaplain will provide spiritual and emotional support for the patients, families and staff of Day Kimball Healthcare in accordance with their needs. The Chaplain also provides spiritual support to Hospice and Palliative Care of Northeastern CT patients and their families. ESSENTIAL RESPONSIBILITIES: * Primary responsibility for the provision of pastoral care for in-patients, oncology, emergency room patients and Hospice and Palliative Care patients and their families * On call as needed * Responds to codes and emergencies when in house and when on call * Charts in-patient and oncology visits appropriately * Makes sure that Hospice pastoral visits are appropriately recorded in the electronic chart * Provides for and models confidentiality in all areas of patient/family and staff support * Assists the Coordinator in the preparation of the Annual Report * Assists in planning and coordinating continuing education programs for community clergy, staff, volunteers, and laity as needed * Supports and ministers to staff as requested and needed, including attending departmental meetings when asked and participating in Critical Incident Debriefing when necessary * Assists Coordinator in acting as liaison with Roman Catholic priests and Eucharistic Ministers * Covers all clinical areas pastorally * Optional: Works as a Notary Public and as an educator and advocate with patients and families and medical staff regarding Living Wills and Advance Directives * Completes annual departmental quality initiatives, including filing paperwork and reports * Completes annual departmental training JOB DUTIES/RESPONSIBILITIES: Promotes the Pastoral Care Services Department's mission, which is to provide spiritual care and nurture to those who provide services to, and those who receive services from Day Kimball Healthcare and Hospice and Palliative Care of NE CT. Completes an initial spiritual assessment as required by Medicare Conditions of Participation guidelines. 2. Based on assessment and patient/family wishes, provide spiritual support and interventions in nonjudgmental manner to patients and families in various life situations and crisis circumstances through follow-up visits when appropriate. 3. Provides crisis intervention to assist patients and their families when dealing with trauma and grief due to loss of life. 4. Participates in interdisciplinary rounds on all units as an integral partner of the health care team. Participates in the interdisciplinary team conferences and assists in developing and reviewing care plans 5. Participates in patient/family meetings to facilitate decision making concerning, advance care planning and end-of-life issues. 6. Provides grief and bereavement support, sacramental ministry, and end of life care to patients, families and staff. 7. Serve as a liaison with notification and involvement of patient/family's own spiritual community at patient/family's request. Assists with funeral and memorial services when requested. 8. Communicates relevant patient information with other disciplines through the use of referral systems, chart entries and other mechanisms in the clinical setting. 9. Documents all contacts per procedure including visits, telephone calls, assessments, interventions, responses to interventions, and communications. 10. Maintains professionalism with colleagues, peers, patients and families. Puts forth a positive agency image. 11. Serves as a resource to staff and provides in-services to the staff and volunteers regarding differing theological points of view, rituals, customs and beliefs and the role of the chaplain. 12. Participates in patient care conferences, bereavement meetings, training programs and other events as appropriate and needed. 13. Knowledge and understanding of what to report to the CEO or Chief Compliance Officer when concerns of corporate compliance arise. 14. Adheres to the ethical standard set forth by Hospital policy and chaplaincy professional organizations. 15. Ensures compliance within guidelines set forth by regulatory agencies (DPH, ERISA etc.) and compliance with policies and procedures. 16. Practices confidentiality principles set by the agency and federal HIPAA/HITECH guidelines. 17. Participates in all mandatory in-services. Physical Requirements: 1. This position shall be occupied by a contract employee under the conditions outlined in this service agreement. This agreement shall not contain any benefit package. This position shall report to the Interfaith Coordinator, consistent with the needs of the agency. This contract employee shall work under the supervision of Interfaith Coordinator to plan, staff and implement worship services, and provide spiritual counseling for inmates of the Jewish faith through knowledge of sacred texts. HopeHealth is seeking an experienced Chaplain The Chaplain actively participates as part of an interdisciplinary team to coordinate and provide spiritual care to patients, families and staff, which are accepting, affirming and respectful of diverse belief systems and spiritual practices. This is a per diem position What will a Chaplain do? Provides spiritual/pastoral assessment through contact with patient/family within five (5) days of admission, per patient need. Provide spiritual/pastoral care in accordance with the ongoing wishes and needs of the patient/family. Develop and implement an individualized spiritual/pastoral plan of care according to goals and wishes of patient/family. Conduct services of prayer, worship and rituals for patients/families, staff and community members as appropriate. Offer patients/families of different philosophies and religious beliefs opportunities to discuss and share their thoughts, feelings, beliefs and values. Document spiritual counseling, and other interventions, provided in the patient's clinical record, according to HPC-RI procedures and policies. Work collaboratively as a member of the Interdisciplinary Team(s) to ensure patient/family needs are met, continually evaluating patient response to care. Facilitate linkage to community clergy or other faith-based leaders as requested by patient/families. Active participation in Interdisciplinary Team meetings, staff meetings, and other meetings as requested and expected for the position. Work collaboratively with Bereavement to plan HPC-RI memorial services and other organizational outreach activities. What are the qualifications for a Chaplain?
