The Acute Care Chaplain works within the healthcare continuum providing emotional, spiritual, and religious care for patients, families, and staff. The Chaplain is responsible to utilize effective communication skills with all patients, volunteers, and staff in a timely, concise, clear, and positive manner.
This position adheres to the Joint Commission standards of Spiritual Care and the Common Standards of Practice for Professional Chaplains and is responsible for the following:
* Patient and caregiver experience is improved through a variety of means including active listening, supportive dialogue, prayer, and meditation.
* Chaplains provide support during a wide variety of clinical scenarios including chronic illness, sudden changes in health status, adjustments to new and chronic illness, and end-of-life situations (including codes whenever possible). They coordinate with local clergy for religious rituals and sacraments as needed and may assist with advance directive preparation and advance care planning.
* Chaplains may lead or coordinate Intermountain caregiver self-care activities, including individual work with caregivers, staff outreach, and group support activities.
* This position consults and collaborates with other members of the interdisciplinary team and contributes to the patient plan of care as appropriate, providing a spiritual perspective on ethical decision-making related to treatment and other health care issues as appropriate.
* Chaplains demonstrate continued professional growth and development through participation in education programs and review of current literature and develop, lead, and/or participate in appropriate support groups as appropriate.
* These duties are performed in a manner that assures that all documentation meets requirements for accuracy and timeliness, and reflects services provided, billing requirements, patient/caregiver needs, and confidentiality. Chaplains maintain confidentiality of business and health information in accordance with HIPAA and state regulations, and Intermountain policies.
Minimum Qualifications
* Bachelor's degree at an accredited school or an equivalent four years of CPE and/or Chaplain work experience.
* Two or more units of Clinical Pastoral Education (CPE) . (Any CPE organization)
* Endorsement training from chaplain's faith organization or a certifying body.
* Maintain ordination/commissioning and endorsement from individual's faith background. Verified upon hire and every five years.
* Demonstrated familiarity with and respect for spiritual, religious and cultural diversity.
* Demonstrated excellent organizational and interpersonal skills.
* Demonstrated ability to communicate effectively orally, in writing, using technology.
* Willing to work as part of a team in an interfaith, inter-spiritual, interdisciplinary multicultural setting.
Preferred Qualifications
* Master in Divinity or a related field
* Four or more units of Clinical Pastoral Education (CPE)
* Board Certified Clinical Chaplain certification from a recognized Chaplaincy program (Required within 2 years of hire APC or SCA)
* One or more years of experience as a professional Chaplain in a healthcare setting.
* Experience working with Palliative Care/Hospice patients.
* Ability to understand, speak, write and read the Spanish language

* Education: Degree from an accredited college or university in Biblical or Religious counseling/studies preferred.
* Experience: Past experience in pastoral or ministry field required. One year of related experience, with a knowledge of psychiatric patient care techniques with understanding of mental illness and chemical dependency (12 step program) preferred.
* Licensure: Must maintain a valid driver's license in the applicable state.

The Acute Care Chaplain works within the healthcare continuum providing emotional, spiritual, and religious care for patients, families, and staff. The Chaplain is responsible to utilize effective communication skills with all patients, volunteers, and staff in a timely, concise, clear, and positive manner.
**This position adheres to the Joint Commission standards of Spiritual Care and the Common Standards of Practice for Professional Chaplains and is responsible for the following:**
+ Patient and caregiver experience is improved through a variety of means including active listening, supportive dialogue, prayer, and meditation.
+ Chaplains provide support during a wide variety of clinical scenarios including chronic illness, sudden changes in health status, adjustments to new and chronic illness, and end-of-life situations (including codes whenever possible). They coordinate with local clergy for religious rituals and sacraments as needed and may assist with advance directive preparation and advance care planning.
+ Chaplains may lead or coordinate Intermountain caregiver self-care activities, including individual work with caregivers, staff outreach, and group support activities.
+ This position consults and collaborates with other members of the interdisciplinary team and contributes to the patient plan of care as appropriate, providing a spiritual perspective on ethical decision-making related to treatment and other health care issues as appropriate.
+ Chaplains demonstrate continued professional growth and development through participation in education programs and review of current literature and develop, lead, and/or participate in appropriate support groups as appropriate.
+ These duties are performed in a manner that assures that all documentation meets requirements for accuracy and timeliness, and reflects services provided, billing requirements, patient/caregiver needs, and confidentiality. Chaplains maintain confidentiality of business and health information in accordance with HIPAA and state regulations, and Intermountain policies.
**Minimum Qualifications**
+ Bachelor's degree at an accredited school or an equivalent four years of CPE and/or Chaplain work experience.
+ Two or more units of Clinical Pastoral Education (CPE) . (Any CPE organization)
+ Endorsement training from chaplain's faith organization or a certifying body.
+ Maintain ordination/commissioning and endorsement from individual's faith background. Verified upon hire and every five years.
+ Demonstrated familiarity with and respect for spiritual, religious and cultural diversity.
+ Demonstrated excellent organizational and interpersonal skills.
+ Demonstrated ability to communicate effectively orally, in writing, using technology.
+ Willing to work as part of a team in an interfaith, inter-spiritual, interdisciplinary multicultural setting.
**Preferred Qualifications**
+ Master in Divinity or a related field
+ Four or more units of Clinical Pastoral Education (CPE)
+ Board Certified Clinical Chaplain certification from a recognized Chaplaincy program (Required within 2 years of hire APC or SCA)
+ One or more years of experience as a professional Chaplain in a healthcare setting.
+ Experience working with Palliative Care/Hospice patients.
+ Ability to understand, speak, write and read the Spanish language

* Completes spiritual needs assessments and formulates interventions strategies based on professional best practice guidelines for addressing spiritual care needs
* Actively collaborates with all members of the treatment team, furthering team-based understanding of significant spiritual needs and spiritual/emotional factors contributing to the patient's well-being.
* In coordination with other IDT members, provides patient/family/staff support in times of crisis, death or difficult diagnoses per hospital and professional protocols.
* Provides spiritually appropriate counseling or guidance in individual and group settings, affirming the dignity, diversity, and autonomy of an individual's own orientation to spirituality and/or religion.
* Conducts and/or assists in coordination of individual, communal, and/or public religious rituals including prayer, spiritually-informed mindfulness/centering, baptisms, weddings, seasonal worship services, and at end-of-life.
* Liaises between patients/families and community-faith leaders to coordinate tradition-specific spiritual care.

Job Summary The Hospice Chaplain is a key member of the hospice care team, helping to coordinate and implement spiritual support activities that reflect the beliefs and values of clients and families. Responsibilities
Perform spiritual assessments and risk assessments of complicating bereavement factors
Attend interdisciplinary team meetings and collaborate with team members to include spiritual needs in the plan of care
Refer to or coordinate with local clergy as requested by the client and family
Coordinate care with other professionals to meet the emotional and psychological needs of the client and family
Communicate effectively and respectfully with a diverse client population and co-workers
Assist families with funeral and memorial services when indicated
Help lead bereavement services for survivors
Submit required documentation in a timely manner
Qualifications
Preferred training includes a bachelor's degree in religious studies or a related subject and a master's degree in theology, divinity or pastoral studies. Hospice chaplain certification is also a plus.
Current CPR with BLS card
Valid driver's license, at least state minimum auto insurance, and an operational vehicle
Possess or obtain a good understanding of Federal, State, and local laws and regulatory guidelines governing hospice operations
Great organization skills and an ability to work with minimal supervision
Ability to remain calm in emergency or crisis situations, and to be close to clients who may be actively dying
Commitment to adhere to professional boundaries
Computer/Technology literacy and experience documenting on an Electronic Medical Record (EMR) system
Physical Requirements
Duties require extensive standing, walking, and sitting
Requires lifting, positioning, pushing, and/or assistance with transferring patients
Requires frequent reaching, stooping, bending, kneeling, and crouching
