Requirements:
* Must be a graduate of an accredited seminary or school of theology, BD, M.Div. or equivalent theological degree
* Must have experience working with death and dying, individuals/family/caregiver
* Hospice experience preferred
* Working knowledge of Microsoft Word, Excel, PowerPoint, and EMR systems.

The Chaplain actively participates as part of an interdisciplinary team to coordinate and provide spiritual care to patients, families and staff, which are accepting, affirming and respectful of diverse belief systems and spiritual practices.

What will a Chaplain do?
Provides spiritual/pastoral assessment through contact with patient/family within five (5) days of admission, per patient need.
Provide spiritual/pastoral care in accordance with the ongoing wishes and needs of the patient/family.
Develop and implement an individualized spiritual/pastoral plan of care according to goals and wishes of patient/family.
Conduct services of prayer, worship and rituals for patients/families, staff and community members as appropriate.
Offer patients/families of different philosophies and religious beliefs opportunities to discuss and share their thoughts, feelings, beliefs and values.
Document spiritual counseling, and other interventions, provided in the patient's clinical record, according to HPC-RI procedures and policies.
Work collaboratively as a member of the Interdisciplinary Team(s) to ensure patient/family needs are met, continually evaluating patient response to care.
Facilitate linkage to community clergy or other faith-based leaders as requested by patient/families.
Active participation in Interdisciplinary Team meetings, staff meetings, and other meetings as requested and expected for the position.
Work collaboratively with Bereavement to plan HPC-RI memorial services and other organizational outreach activities.
What are the qualifications for a Chaplain?
Master's Degree in religious or theological studies required
One to two units of Clinical Pastoral Education or equivalent training, education and experience
One to three years' experience for Chaplain, three to five years' experience for Senior Chaplain, in community service
Must be comfortable with prayer in an interfaith environment and respectful of all religious beliefs/spiritualties
Knowledge and experience with grief and end of life issues preferred

The Staff Chaplain will provide spiritual support and services to patients and caregivers and the Dana-Farber community. The Staff Chaplain is required to be 100% on-site. Responsibilities
* Visit patients who request or are referred by a member of their DFCI care team for religious, spiritual, or existential support at any point in their DFCI care journey.

* Document visits in the electronic medical record and refer to other members of the care team as appropriate.
* Lead interfaith gatherings, services and ritual for patients/caregivers and staff as requested.

A minimum of one unit of Association of Clinical Pastoral Education, Inc. (ACPE)-accredited CPE.
Note a degree received from an accredited college/university is preferred. If the degree is received from a non-accredited college or university, AVPO approval is required prior to hire.
Additional Experience:
Two years of active work in spiritual care ministry/ program, one unit of pastoral education (per state requirements, if indicated), and demonstrated ability to work with patients, families, and community spiritual leaders of various denominations/beliefs is required.
Three years of active work in hospice spiritual care support and one year of clinical pastoral education is preferred.
