The chaplain ministers to hospital/hospice patients, families, medical staff, and the community at large. The chaplain serves as a member of the Medical Center's multi-disciplinary team in providing comprehensive holistic health care. The chaplain performs pastoral care, educational and counseling services, and administrative duties. This position assists with selection, orientation, supervision, and continuing education of per diem chaplains. The Chaplain also supports Clergy and their associations to facilitate pastoral support and visitation of hospitalized parishioners. This position assists patients and in training of staff by providing information about advance medical directives and by assisting with the execution of the advance medical directive as requested by patients. This position practices the Heart of Hamilton philosophy of the Hamilton HealthCare System culture. Qualifications JOB QUALIFICATIONS Education: Bachelors Degree, Master of Divinity or equivalent from a theological seminary. 4 units of Clinical Pastoral Education (CPE) preferred. 2 unit of CPE required. Licensure: Ordained minister seeking denominational endorsement Experience: One-year clinical pastoral educational residency or the equivalent, healthcare experience preferred Skills: Good verbal and written skills. Demonstrates therapeutic capabilities of intervening during both crisis and non-crisis situations. Demonstrates compassion and caring skills for patients, families, associates, and medical staff, and an ability to assist patients with advance medical directives. Must be able to demonstrate the knowledge and skills necessary to provide care appropriate to the age of the patient served. Wesley Woods community chaplains provide a unique intersection of church and older adult residential living. The community chaplain acts as a spiritual guide and pastor for the residents of Wesley Woods communities, offering a ministry of presence, prayer, worship, group study, fellowship, and pastoral care in a setting where people are aging and thriving with grace and dignity. In addition to being a pastor in the community, the community chaplain will connect local United Methodist churches (and other churches and faith communities as appropriate) in mission and ministry with residents and staff. The community chaplain will take the initiative to cultivate a culture of mission with older adults that invites local churches into a stewardship and mission relationship with Wesley Woods. Under the supervision of the Director of Pastoral Care and in coordination with residential staff (or Residential Administrator), the community chaplain will be part of a team effort to provide for the ongoing care of older adults who may need additional resources to remain in their homes. ACPE Clinical Pastoral Education a plus. Professional knowledge of medical ethics and ability to counsel and support residents, families, and staff in difficult situations. Experience in pastoral ministry with older adults in both church and hospital settings. Experience in cultivating, training and coordinating volunteers in a community or church setting. Excellent verbal and written communication skills and strong interpersonal relational skills are required for participation in a team environment. Experience in a retirement community preferred. Knowing where the patient is coming from helps us determine what they need to feel comfortable, safe, and healthy in their own home. Essential Functions: Conducts assessments of hospice patients and family spiritual needs. Ensures that appropriate spiritual services are provided in a timely manner by either providing such to the patient/family or coordinating the provision of services with community clergy or spiritual counselors. Provides direct spiritual services to patients/families according to their belief system and practice. Participates in the development of the interdisciplinary group (IDG) plan of care to meet identified spiritual needs. Conducts religious services for patients/families as well as hospice and facility staff as requested. Serves as a liaison and support to community clergy and spiritual counselors. Documents services provided and/or ongoing communication with community clergy and spiritual counselors in a timely manner. Maintains records of spiritual services utilization and related activities as directed for quality assessment and performance improvement (QAPI), hospice program development, and policies and procedures review and revision. Provides consultation, education, and support regarding spiritual issues and care to the IDG members and facility staff. Recruits an adequate number of community clergy and/or spiritual counselors to meet patient/family needs. Acts as the hospice spiritual services community liaison by developing community contacts and offering hospice education to a variety of clergy, counselors, and congregations. Provides funeral or memorial services for patients as requested. Plans periodic memorial services to meet the needs of IDG members, volunteers, facility staff, and community clergy/spiritual counselors working with the IDG. Primarily responsible for conducting or arranging the memorial service associated with the hospice bereavement program as requested. Assists in the supervision of spiritual care volunteers when assigned to patients/families. May have access to personal health information (“PHI”) necessary to fulfill the above duties and responsibilities. Minimum Education & Experience Requirements: Graduate of accredited seminary or school of theology (BD, MDiv or equivalent theological degree), or certification in Level I and II of Clinical Pastoral Education (from an accredited ACPE Center). Experience working with death and dying, terminal individuals, and their families or caregivers. Hospice experience preferred. Skills, Abilities & Knowledge Requirements: Knowledge of the hospice philosophy of care. Good verbal and written communication, and organizational skills. Able to work as a member of the IDG. Able to effectively communicate with clinical and non-clinical employees, patients and family/caregivers. Able to perform and prioritize multiple functions or tasks. Able to effectively deal with change. Able to provide proof of valid driver's license and proof of valid auto liability insurance if assignment includes driving own vehicle. You will be responsible for the spiritual services provided to hospice patients and families. Participate in developing the interdisciplinary care plan. Visit patients/families identified as appropriate for spiritual care services and provides direct spiritual support, counsel, prayer, and ministry as appropriate. Assist members of the interdisciplinary team in understanding the significant spiritual concerns related to the patient/family response to dying. Serve as liaison and resource for community spiritual leaders in dealing with hospice patient/family. About You The candidate must meet ONE of the following requirements: BS or BA in theology, religion, human services, counseling, psychology, or sociology from a college or university and/or as required by state-specific regulations. A Master's Degree in counseling, psychology, theology or divinity from a college or university and/or as required by state-specific regulations. (Preferred.) A minimum of one unit of Association of Clinical Pastoral Education, Inc. (ACPE)-accredited CPE. Note a degree received from an accredited college/university is preferred. If the degree is received from a non-accredited college or university, AVPO approval is required prior to hire. Additional Experience: Two years of active work in spiritual care ministry/ program, one unit of pastoral education (per state requirements, if indicated), and demonstrated ability to work with patients, families, and community spiritual leaders of various denominations/beliefs is required. Three years of active work in hospice spiritual care support and one year of clinical pastoral education is preferred.
