RESPONSIBILITIES
As a Navy Chaplain, your job spans a broad range of duties. You will support fellow servicemembers during their most joyful and most difficult moments. Your responsibilities might be to:

Job Summary:
The Hospice Chaplain under the supervision of the Clinical Director works as an integral part of an interdisciplinary team and provides spiritual, emotional and some bereavement care to support hospice patients, clients, and families in facilities and in-home settings. Essential Duties:
The chaplain provides pastoral/spiritual care to patients and/or families.
Visits and ministers with patients. Works with the Clinical Director and other appropriate persons in developing a spiritual care continuum of care appropriate for hospice patients/families.

At least four units of Clinical Pastoral Education (CPE). Candidate must be Board Certified as a Chaplain or eligible for Board Certification under the Association for Clinical Pastoral Education (ACPE) or the College of Pastoral Supervision and Psychotherapy (CPSP).
Additional qualifications include excellent communication and pastoral skills. Must have a high degree of emotional energy and mental alertness and ability to deal with the challenge of supporting patients and loved ones who experience pain, suffering, death and grief.
Good organizational and decision-making skills.
Computer Literacy and ability to perform extensive and timely charting timely using the Company's EMR System.
Licensure:

Strong interpersonal skills. Ability to accept and work with diverse ages, different lifestyles, cultures, socio-economic and religious backgrounds, spiritual beliefs and values.
Professional knowledge and clinical ability sufficient to develop and maintain collegial and collaborate relationships with physicians, nurses, social workers, clients and other members of the healthcare team.

As a Hospice Chaplain (part-time), you will:
+ You will report directly to the Administrator or Executive Director.
+ You will be responsible for the spiritual services provided to hospice patients and families.
+ Participatein developing the interdisciplinary care plan.
+ Visit patients/families identified as appropriate for spiritual care services and provides direct spiritual support, counsel, prayer, and ministry as appropriate.

About You
The candidate must meet ONE of the following requirements:
+ BS or BA in theology, religion, human services, counseling, psychology, or sociology from a college or university and/or as required by state-specific regulations.
+ A Master's Degree in counseling, psychology, theology or divinity from a college or university and/or as required by state-specific regulations. (Preferred.)
+ A minimum of one unit of Association of Clinical Pastoral Education, Inc. (ACPE)-accredited CPE.
+ Note a degree received from an accredited college/university is preferred. If the degree is received from a non-accredited college or university, AVPO approval is required prior to hire.
Additional Experience:
+ Two years of active work in spiritual care ministry/ program, one unit of pastoral education (per state requirements, if indicated), and demonstrated ability to work with patients, families, and community spiritual leaders of various denominations/beliefs is required.
+ Three years of active work in hospice spiritual care support and one year of clinical pastoral education is preferred.

The Hospice Chaplain has a true calling and is a passionate advocate for the patients we serve. The Choice Chaplain is responsible for assessing, providing, and meeting the spiritual care in the home to the terminally ill patients and their families. Makes reasonable efforts to facilitate visits by local clergy, pastoral counselors or other individuals who can support the patient's spiritual needs. Participates as a core member of the Interdisciplinary Group.
As a Hopestone Hospice Chaplain, you will:
Assess hospice patients and support systems to identify spiritual needs as evidenced by POC, documentation, clinical records, and IDG conference minutes.
