REWARDING WORK YOU WILL DO:
Provides spiritual counseling and emotional support to patients and families/caregivers during the course of care and at the initial phase of bereavement. The position works collaboratively with the Interdisciplinary Team/Group (IDT/IDG) as well as with facilities and agencies in the community.
Completes initial spiritual assessment. Develops spiritual care plan and provides spiritual care from admission to death or discharge.
Schedules and visits patient and families to provide spiritual care and support. Provides education, direct spiritual counsel, prayer and ritual/sacramental ministry, as appropriate and requested, regarding the dying process and end of life issues.
Acts as a liaison between the patient, family and their faith community when appropriate.
Participates in IDT/IDG meetings to identify, evaluate and communicate feedback on the religious and/or spiritual needs of the patient and family as part of the Plan of Care (POC).
Communicates risk indicators to Bereavement Services as appropriate.
Assists with final arrangements, such as mortuary selection, planning memorial/funeral services, contacting clergy, sacraments/rites or other services according to patient and family goals. When requested, conducts sacraments/rites, worship, funeral or memorial services.
Provides education about eye, body or tissue donation options. Helps facilitate donation process.
WHAT WE ARE GOING TO LOVE ABOUT YOU:
Minimum Education: Master's degree in Divinity or related field. One (1) unit of C.P.E required; two (2) units preferred.
Minimum Experience: One or more years in a spiritual service role.
Required License: Ordination by a religious organization or endorsement letter of good standing by a religious organization. Valid Colorado state driver's license and proof of auto insurance. Current CPR certification based on individual affiliate requirements.
PHYSICAL REQUIREMENTS:
Ability to lift/push/pull minimum of 30 lbs.

Essential Functions and Responsibilities
•Serves as a messenger and conveyor of faith, mission, and purpose.
•Interacts with companies, their employees, and the community in a manner that exhibits character through personal and spiritual disciplines.
Ministry
•Makes brief visits to company worksites regularly (usually weekly) to interact with employees to build a relationship of trust and friendship motivated by the Chaplain's deep Christian faith.
•Visits employees or immediate family members wherever care can be expressed and help given: hospitals, nursing homes, funeral homes, family residences, or other sites.
•May provide confidential pastoral discussions for problem issues of employees and their immediate family members including, but not limited to family matters, divorce, serious illness, care of aging parents, death and grief recovery, parenting, financial situations that may include debt discussions, budgeting, and other life issues related to financial stress, as well as any other personal issues. Provides referral service and acts as coordinator for specialized assistance to employees and/or immediate family members with specific needs.
•May assist in the planning, conduct or attend funerals for employees or immediate family members, including follow-up support and encouragement to immediate family members during the grief period.
•May make jail visits to employees and immediate family members.
•Works with companies to serve notifications of death and serious injuries to families and other employees, encouraging those impacted by various tragedies.
•May provide post-termination care for laid-off or terminated employees in order to foster a smooth transition to another work environment.
•May facilitate, but not lead, spiritual enrichment activities, as an employee-sponsored and led activity will usually be more fruitful than a chaplain led event.
•Provides, as appropriate/requested, literature and other resources to assist company employees with life issues.
•Represents companies to clients/customers where it would be appropriate to ministry services involved (i.e., serious illness, accidents, traumatic events, and funerals).
•Participates in new employee orientation to explain the Employee Care Service and the role of the Chaplain Team.
•With Executive Director of Operations (EDO) permission, responds positively and assertively to other duties and services requested by executive leadership of companies, within the scope of the Letter of Agreement between MCHAP Chaplains and the company.

The expertise and experiences you'll need to succeed:
Bachelor's Degree in spiritual care, arts, or equivalent degree from college/university or institution accredited by the applicant's institutional faith body. Completion of Roman Catholic deacon training and ordination is also acceptable if applicant does not have a bachelor's degree. Formal study in a theologically related field is preferred.
Bachelor's Degree in unrelated field is allowed if applicant has a Master's Degree in spiritual care related field or equivalent, or is currently enrolled in a such a Master's Degree program.
Four Units (quarters) of CPE (Clinical Pastoral Education). At hire, applicant must have at least one unit of CPE or enroll in our internal Clinical Pastoral Orientation and complete 1 unit of CPE within one year. If the chaplain has less than 4 units of CPE, they must agree to complete additional units in a CPE program (one unit at least every six months until fulfilling the requirement of 4 units) from a nationally recognized accrediting agency listed by the U.S. Department of Education. Fifty hours of continuing education in chaplaincy-related topics are required on an annual basis, except when enrolled in CPE or preparing for certification.
Affiliation with an institutional faith body.
Current Board Certified Chaplain documentation. Chaplains who have completed 4 units of CPE are expected to become board certified within 12 months. Exceptions must be approved by their manager. Board certification organizations include\: National Association of Catholic Chaplains, Board of Chaplaincy Certification Inc, Association of Professional Chaplains, Spiritual Care association (4 units), The Association of Certified Christian Chaplains, or equivalent. As a Hospice Chaplain you need to know how to:
Partners with members of the Hospice and post-acute care services teams to provide a multidisciplinary approach to care and planning.
Conduct comprehensive assessments/reassessments of spiritual needs for spiritual care plan, facilitate interventions and support for patients and families within required timeline.
Provides services to the patient/caregiver in accordance with the plan of care.
Participates in multidisciplinary patient rounds, care conferences, and consultations.

As a member of the interdisciplinary team the Interfaith Chaplain is responsible to the Director for the provision of spiritual support to patients and families in keeping with the patient/familys desire and belief system. The hospice

Job description: A medical team clinician specializing in helping patients, families, residents and staff access their spiritual resources to facilitate healing and/or end-of-life adaptive transitions. Qualifications

organization focusing the care of hospice, home health, or senior living. Possible examples might include: Grief Recovery Institute, NHPCO, HFA, Center for Loss and Life Transition, and the American Academy of
Bereavement (AAB).
30 hours per CY of continuing education in chaplaincy related topics are required on an annual basis, except when enrolled in CPE, completing a master's program, engaged in RCC diaconate formation, or
preparing for Board Certified Chaplain (BCC or equivalent) credentialing or RCC ordination. (cf. Mountain Division ICCE guidelines)

Essential Functions and Responsibilities

Ministry
•Makes brief visits to company worksites regularly (usually weekly) to interact with employees to build a relationship of trust and friendship motivated by the Chaplain's deep Christian faith.
•Visits employees or immediate family members wherever care can be expressed and help given: hospitals, nursing homes, funeral homes, family residences, or other sites.
•May provide confidential pastoral discussions for problem issues of employees and their immediate family members including, but not limited to family matters, divorce, serious illness, care of aging parents, death and grief recovery, parenting, financial situations that may include debt discussions, budgeting, and other life issues related to financial stress, as well as any other personal issues. Provides referral service and acts as coordinator for specialized assistance to employees and/or immediate family members with specific needs.
•May assist in the planning, conduct or attend funerals for employees or immediate family members, including follow-up support and encouragement to immediate family members during the grief period.
•May make jail visits to employees and immediate family members.
•Works with companies to serve notifications of death and serious injuries to families and other employees, encouraging those impacted by various tragedies.
•May provide post-termination care for laid-off or terminated employees in order to foster a smooth transition to another work environment.
•May facilitate, but not lead, spiritual enrichment activities, as an employee-sponsored and led activity will usually be more fruitful than a chaplain led event.
•Provides, as appropriate/requested, literature and other resources to assist company employees with life issues.
•Represents companies to clients/customers where it would be appropriate to ministry services involved (i.e., serious illness, accidents, traumatic events, and funerals).
•Participates in new employee orientation to explain the Employee Care Service and the role of the Chaplain Team.
•With Executive Director of Operations (EDO) permission, responds positively and assertively to other duties and services requested by executive leadership of companies, within the scope of the Letter of Agreement between MCHAP Chaplains and the company.

As a Hospice Chaplain, you will:
You will report directly to the Administrator or Executive Director.
You will be responsible for the spiritual services provided to hospice patients and families.
Participate in developing the interdisciplinary care plan.
Visit patients/families identified as appropriate for spiritual care services and provides direct spiritual support, counsel, prayer, and ministry as appropriate.

A minimum of one unit of Association of Clinical Pastoral Education, Inc. (ACPE)-accredited CPE.

Additional Experience:
Two years of active work in spiritual care ministry/ program, one unit of pastoral education (per state requirements, if indicated), and demonstrated ability to work with patients, families, and community spiritual leaders of various denominations/beliefs is required.
