Richmond Behavioral Health is seeking PRN Licensed Practical Nurses (LPN) to join our Medication Assisted Opioid Treatment Program. The selected individual will preformintermediate human support work in the care and treatment of patients, managing, preparing and monitoring medications, administering observed dosed medication, injections, obtaining lab specimens, preparing and maintaining records and files, and related work as apparent or assigned. Work is performed under the limited supervision of the assigned supervisor.
Essential Functions
Assesses and evaluates clients; obtains and reviews pertinent medical data; collects vital signs and specimens; monitors and documents patients response to treatment.
In collaboration with the Pharmacist, manages, sorts and prepares medications for distribution to patients; evaluates, reviews and monitors medication compliance.
Participates in diversion control efforts, such as patient call-backs and medication counts; documents results in patient record and reports concerns to Registered Nurse and case manager assigned to patient.
Administers and observes ingestion of oral medication, injections, immunizations, and medications with training and supervision; assist the physician as needed.
Delivers medication and to patients in residential treatment and observes dosing.
Assists Registered Nurse with triage and crisis intervention as needed. Provides crisis intervention services as necessary.
Draws venous blood, finger stick, urine and other routine laboratory work; obtains and processes specimens; sends to the laboratory for analysis.
Consults with prescriber, lead nurse, case manager, team members and pharmacies on new orders, changes, pre-authorizations and lab results; reviews and verifies discharge orders.
Prepares and maintains patient charts, logs and records, and utilizes electronic health records and other software to document dosing and other medical and pharmacy concerns
Assists the Registered Nurse in preparing supply orders, tracks and stocks supplies and medications.
Participates in training in the use of naloxone to reverse opioid overdose.
Attends meetings, seminars and training as required.
Position Requirements
Knowledge, Skills and Abilities
Thorough knowledge of practical nursing techniques and the care and treatment of all types of clients; considerable knowledge of sanitation, personal hygiene, basic health and safety practices; skills in preparing and administering medications and general knowledge of proper dosage, effects and contradictions, especially methadone and other medications used in the treatment of opioid use disorder (OUD); skill in the operation of general and specialized equipment used in the area of assignment; ability to make astute observations of patient's conditions and to report changes accurately; ability to understand, interpret, and relate oral and written instruction; ability to solve problems within scope of responsibility; ability to establish and maintain effective working relationships with clients, medical personnel, other employees and the general public. Knowledge of addiction and the symptoms of drug and alcohol use, especially opioids, and impact of methadone as a medication to treat OUD. Ability to utilize Electronic Health Records (EHR) and other software to track patient progress and document dosing. Excellent oral and written communication skills, and ability to relate to individuals from diverse backgrounds. Ability to work as a team member with a variety of medical and behavioral health professionals.
Education and Experience
Graduation from a State accredited program of practical nurse education and moderate experience as a practical nurse in a behavioral health setting.
Special Requirements
Possession of a current license as a Practical Nurse issued by the Commonwealth of Virginia Board of Nursing.
CPR certification.
Valid driver's license in the Commonwealth of Virginia.

Wound Care Nurse (RN) - Inpatient Wound Ostomy

PRN/Supplemental - Day shift (8am-4:30pm, some weekends)
Under the supervision of the Clinical Manager, performs the primary function of an RN in assessing, planning, implementing and evaluating the care of patients in the Wound Healing Center. Is responsible for meeting the established WHC Standards of Nursing Practice in the management of all assigned patients. Assists physicians with the development and implementation of wound treatment plans and participates in performance improvement review activities. Acts within the guidelines of federal and state laws, JC AHO Standards and professional and ethical practice. Also administers hyperbaric oxygen therapy and diagnostic testing to patients of the Department of Hyperbaric Medicine, under the supervision of a hyperbaric physician. All therapeutic and diagnostic modalities will be ordered by the hyperbaric physician.
Role Specific Competencies
1. Clinical - Assessment
Initiate a nursing assessment of patient's condition upon admission.
Assess and interpret reports communicating findings with the assigned physician.
Recognize and assess initial alterations in the patient's body systems and verbalize this to other members of the health care team.
Organize assessment data for accuracy, completeness and confidentiality.

Complete the 4 week reviews in a timely manner and ensure all recommendations to the plan are activated in the physician orders.
Participate in quality improvement/staff meetings.
4. General
Function as a case manager for a defined patient population.
Coordinate office visits, pre-procedure work-ups, and patient education with the multidisciplinary team.
Foster continuity of care between WHC and care taker (home, hospital, HH etc)
Coordinate the use of clinic staff, exam room space, and supplies.
Prepare for clinic to enable accurate and complete updates to the physician.
Assist others within the clinic.
Triage telephone calls and assist with scheduling.

5. HBO
Demonstrate professionalism and commitment to the mission statement of the Department of Hyperbaric Medicine, as well as Client Hospital at all times.
Demonstrate a working knowledge of the policies and procedures of Hyperbaric Medicine.
Perform transcutaneous oxygen monitoring testing as ordered and per departmental policy.
Ascertain that the patient's diagnosis is one of the appropriate approved disorders for Hyperbaric Oxygen Therapy, and that the said diagnosis is clearly entered on the patient's charts/records.
Implement the physician's orders as indicated by the patient's treatment plan.
Educate patient and/or patient's family as to the benefits, risks, contraindications, and safety issues of hyperbaric oxygen therapy before treatment.
Perform blood glucose monitoring as ordered and indicated by patient diagnosis.
Inspect the patient prior to the commencement of each Hyperbaric Oxygen Therapy session to ensure all safety standards are being adhered to, i.e. 100% cotton attire, no makeup, no hairspray, etc.
Perform the hyperbaric treatments and monitor the patient during therapy.
Perform dressing changes and/or wound care within scope of practice, as ordered and participate in clinic when HBO volumes are below par.
Take serial photographs of the wound for reporting of the patient's progress.
Evaluate the patient outcome and meet with the manager and the hyperbaric physician to discuss patient treatment and adjust treatment as necessary.
Consult with hyperbaric physician regarding any adverse reactions.
Document treatment notes and maintain patient treatment/diagnostic records.
Complete the patient's treatment charges, if required.
Perform quality control daily on department glucometer, or as required by policy.
Clean and maintain the equipment according to infection control standards.
Maintain continuous quality improvement duties and chart audits.
Answer telephone inquiries and schedule patient treatment times.
Perform marketing duties as needed, intra- and interdepartmental.
Perform an inventory check, and maintain a record of supplies and laundry items.
Maintain equipment and checks backup air and oxygen supplies on a daily basis.
Perform data entry into the Wound Information Data System and collate reports as requested by the director.
Orient new employees as necessary.
Participate in New Patient Reviews to assess for HBO indicators.
Qualifications and Experience
Current RN licensure in state of Maryland
Minimum of 2 year's med-surg experience. Wound Care experience preferred.

BAYADA Home Health Care is currently seeking an experienced Physical Therapist Assistant, PTA, for a full-time or part-time opportunity performing home health visits for our Montgomery County Senior Living Office. This office services adult and geriatric patients on a per visit basis in Independent and Assisted Living communities throughout Montgomery County.
As a home care Physical Therapist Assistant (PTA) you will assist clients in attaining or maintaining optimal physical well being by providing treatment, tests, client education and appropriate interventions as delegated by a licensed Physical Therapist.
Each Physical Therapist Assistant (PTA) must:
Have current licensure or certification in the state of MD
Have graduated from a two-year college level program approved by the American Physical Therapy Association, as indicated by school transcript or diploma, or
Have two years of appropriate experience as an Physical Therapist Assistant (PTA) and a satisfactory grade on a proficiency exam conducted, approved or sponsored by the U.S. Public Health Service when initial qualification occurred before December 31, 1977
Have at least one years work experience under the supervision of a qualified Physical Therapist.

Maintains thorough documentation of daily activities including nursing, treatment services, crisis intervention, case management, reimbursement, and productivity statistics.

Complete the 4 week reviews in a timely manner and ensure all recommendations to the plan are activated in the physician orders.
Participate in quality improvement/staff meetings.
4. General
Function as a case manager for a defined patient population.
Coordinate office visits, pre-procedure work-ups, and patient education with the multidisciplinary team.
Foster continuity of care between WHC and care taker (home, hospital, HH etc)
Coordinate the use of clinic staff, exam room space, and supplies.
Prepare for clinic to enable accurate and complete updates to the physician.
Assist others within the clinic.
Triage telephone calls and assist with scheduling.
Maintain an open, professional communication line with patients, physicians, and staff.

Demonstrate professionalism and commitment to the mission statement of the Department of Hyperbaric Medicine, as well as Client Hospital at all times.
Demonstrate a working knowledge of the policies and procedures of Hyperbaric Medicine.
Perform transcutaneous oxygen monitoring testing as ordered and per departmental policy.
Ascertain that the patient's diagnosis is one of the appropriate approved disorders for Hyperbaric Oxygen Therapy, and that the said diagnosis is clearly entered on the patient's charts/records.
Implement the physician's orders as indicated by the patient's treatment plan.
Educate patient and/or patient's family as to the benefits, risks, contraindications, and safety issues of hyperbaric oxygen therapy before treatment.
Perform blood glucose monitoring as ordered and indicated by patient diagnosis.
Inspect the patient prior to the commencement of each Hyperbaric Oxygen Therapy session to ensure all safety standards are being adhered to, i.e. 100% cotton attire, no makeup, no hairspray, etc.
Perform the hyperbaric treatments and monitor the patient during therapy.
Perform dressing changes and/or wound care within scope of practice, as ordered and participate in clinic when HBO volumes are below par.
Take serial photographs of the wound for reporting of the patient's progress.
Evaluate the patient outcome and meet with the manager and the hyperbaric physician to discuss patient treatment and adjust treatment as necessary.
Consult with hyperbaric physician regarding any adverse reactions.
Document treatment notes and maintain patient treatment/diagnostic records.
Complete the patient's treatment charges, if required.
Perform quality control daily on department glucometer, or as required by policy.
Clean and maintain the equipment according to infection control standards.
Maintain continuous quality improvement duties and chart audits.
Answer telephone inquiries and schedule patient treatment times.
Perform marketing duties as needed, intra- and interdepartmental.
Perform an inventory check, and maintain a record of supplies and laundry items.
Maintain equipment and checks backup air and oxygen supplies on a daily basis.
Perform data entry into the Wound Information Data System and collate reports as requested by the director.

Qualifications:

Responsibilities:
Assist nursing staff with completion of resident activities of daily living, bathing, dressing, oral care, and other basic needs
Perform vital signs on residents at beginning of each shift and as directed
Answer calls for assistance and observe changes in resident's behavior, emotional state, or physical condition
Complete required documentation in a timely manner
Assist in transporting residents as needed
Provide companionship to residents
Ensure the safety of our residents
Assist with motivating residents to help keep them active and engaged
Maintain a clean and healthy environment
Qualifications:
Must hold a current certification and GNA license in Maryland
Must possess a positive and professional attitude
Ability to work well in a team-oriented environment
Previous experience working in an skilled nursing facility is a plus
Ability to communicate effectively
Ability to remain calm and patient when dealing with residents
Ability to listen attentively is essential
Ability to problem solve and work well under pressure
A willingness to navigate changes in assignments
Ability to show empathy and compassion while interacting with residents to increase their comfort level
