Provide direct and individualized nursing care to assigned residents based on established nursing protocol. Observe and assess for: pain/discomfort, oxygen therapy, all drainage, all dressings, and any incontinent elimination which may have occurred.
Provide ignition and ongoing systematic assessment of the physical, emotional, social, educational, and functional needs of the resident/family through observation, interview, and physical examination.
Interpret assessment data and information to assist in developing a plan of care.
Assure medical orders are transcribed and processed accurately and timely and integrate the medical care plan into the provision of nursing care.
Assure that medication administration is correct, according to the established nursing policies and procedures.
Provide accurate and timely documentation consistent with the resident plan of care.
Administer medication per physician orders. Document properly on MOR. Observe for any adverse reaction.
Assemble resident charts, resident records, and assure all necessary form are completed in a timely manner. Assure all monthly summaries are completed in a timely manner for assigned residents.
Prepare incident/accident reports for residents as needed per policy, notify AL Director, physician, and family post incident per policy and document accordingly.
Assist resident in learning self-care activities.

In conjunction with the patient/family and with the direction of the RN, the RNA provides care based on the individualized patient care plan. The RNA observes and reports patient issues/concerns/problems and on an ongoing basis will report findings to LVN or RN,
*As a member of the healthcare team the RNA implements and incorporates basic patient care activities across the continuum by effectively communication with physicians, staff and other disciplines.
*Responds appropriately to concerns regarding patient safety based on the physical environment, equipment and the patient's behavior.
*Demonstrates the ability to provide care in emergency situations and following established emergency procedures and practices, as appropriate.
*Communicates and follows the organizational chain of command for notification of patient care/service issues, when appropriate.
*Assures that the rights of all patients are respected and maintained by allowing for privacy and dignity in the provision of care.
*Performs and documents heights and weights accurately.
*Provides positioning/splinting equipment for appropriate patients and monitors correct use of the equipment.
*Performs ROM, strengthening exercises and ambulation for residents in the RNA Program.
*Participates in the RNA, IDT and Weight Variance meetings.
*The RNA is aware of the reporting responsibilities and reports suspected cases of abuse or neglect relative to the adult through geriatric rehabilitation patient population as appropriate.
*Reinforces patient/family educational needs as identified by the RN and helps provide individualized basic teaching during the care giving process which considers the functional/developmental age and the needs of the adult through geriatric rehabilitation patient population. The RNA observes and reports the effectiveness of the patient/family teaching through observation of patient responses.
*Demonstrates knowledge of age specific care, including but not limited to, identifying the need for additional safety measures, physiological normal values/readings; and assessing skin integrity, behavior, motor skills and/or activities that place patients at risk as well as communicates effectively in a clear, concise, understandable manner.
