Responsibilties:
Provide in-home, para-professional services necessary for caring for patients' personal needs under the direction of the RN.
Perform and/or assist patient with personal care activities, such as, baths, showers, skin and hair care, and oral hygiene. Assure universal safety precautions and direction on the care plan are followed when providing care. Provide care that meets the patient's needs.
Notify RN of any changes in patient condition observed in a timely manner and document appropriately.
Record all required information on patient chart completely and accurately. Assure documentation is completed per organization protocol.
Assist patient with transfer and ambulating. Perform range of motion exercises. Assure all activities are performed safely and follow the principles of good body mechanics.
Perform general housekeeping of patient's area and change patient's bed linens. Assist family with light housekeeping activities, such as dusting, vacuuming, damp mopping, etc.
Assist client with self-administration of oral medication/s. Remind patient when to take medication/s.
Prepare simple meals in accordance with dietary instructions and assist patient with eating if necessary.
Assist with of foley catheter and colostomy care to ensure proper hygiene. Assist patient with changing colostomy bags. Perform peri care. Measure urine. Assist with ostomy care and assure skin is clean around stoma.
Other duties and/or projects as assigned. Demonstrate flexibility with job responsibilities in all areas.

You will report directly to the Executive Director. You will be responsible for the timely admission of all patients to the program and serve as the lead coordinator of all patient admissions activity.
Overview:
Responsible for contacting the patient and family within 1 hour of referral, and coordinating all admissions within the 3-hour admission Commitment
Processes referrals by obtaining sufficient admission and insurance information in order to ensure appropriate and timely admission of patients
Participates as a member of the marketing team by: actively seeking new referral sources; developing and maintaining positive relationships with referral sources; assist in developing strategies and plans for site and company growth
Ensures maximum reimbursement through initial payer source, benefits, and eligibility verification, authorization processes, and accurate data entry into Company computer system. Completes required payer source verification forms
Provides accurate insurance and admission information to admission staff in order to ensure appropriate disclosure of rights and responsibilities, financial liabilities and benefit plan details to patients and their families
Provides Billing Coordinator, or designee, with insurance and billing documentation as needed for final confirmation prior to admission and/or submission of bill
About You:
High school diploma or equivalent with customer service experienced required
Must have strong communication, organization, negotiation and public relation skills and be proficient with the PC including the MS suite of products.
Hospice or medical office experience preferred
LPN/LV certification or Bachelor's degree in business, social work, psychology or nursing or equivalent business experience preferred

We are currently recruiting for a compassionate Home Health Physical Therapy Assistant (PTA) to reliably serve patients through out Burlington county. This is a full-time, pay per point position.
What you'll do:
As a Jefferson Health at Home by BAYADA Physical Therapy Assistant, you'll use your clinical skills and compassionate heart to treat patients where they want to be the most - in the comfort of their own home. As a Jefferson Health at Home by BAYADA PTA you will:
See a client through their care needs - from hospital discharge to living a safe home life with comfort, independence, and dignity.
Assist clients in attaining or maintaining optimal physical wellbeing by providing treatment, tests, client education and appropriate interventions as delegated by a licensed Physical Therapist.
Continually assess and revise the Physical Therapy - PT care plan, and participate with nursing in the multidisciplinary care plan, as appropriate.
What you should have:
Have current licensure or certification in the state of New Jersey.
Have graduated from a two-year college level program approved by the American Physical Therapy Association, as indicated by school transcript or diploma, or
Have two years of appropriate experience as a Physical Therapist Assistant (PTA) and a satisfactory grade on a proficiency exam conducted, approved or sponsored by the U.S. Public Health Service when initial qualification occurred before December 31, 1977
Have at least one year's work experience under the supervision of a qualified Physical Therapist.
Previous home healthcare experience welcomed, but not required. We will train you.
A calm and reassuring presence, active listening skills, excellent assessment skills and strong written and verbal communication.

Job Summary: Responsible for scheduling all surgical appointments, communicating with patients regarding medication, lab results, and tissue diagnosis, maintaining medical supply inventory, and ensuring compliance with OSHA, CLIA, and HIPAA.
ROOM PREPARATION
At start of the work-day, checks all patient rooms to assure that they are ready for patients, clean and stocked
Fills Cryo containers
PATIENT VISIT
Escorts patients to exam room, documents a brief history as instructed by the physician using EHR system. Assists patient with disrobing if necessary.
Assist physician with exam of patient. Obtain samples for the patient or other supplies that are needed during the visit.
Answers patient questions if possible, refers others to the physician.
Cleans rooms between patients, changing table paper and cleaning tables and counter tops etc. as necessary.
Sets up rooms for procedures with the designated instruments and sterile field. Explains procedure to patient and obtains informed consent. Provides wound care instructions to patient after procedure.
Logs in and out all pathology specimens and orders for other lab work
MAINTENANCE
Transports used instruments from room to instrument maintenance area, keeping safety standards in mind. Cleans instruments, wraps appropriately, and autoclaves.
Checks rooms at end of day to make certain they are stocked for the next day.
Maintains inventory of medical and surgical supplies and places orders appropriately.
Keeps nursing station in order, making certain that all expired medications are appropriately discarded. Calls pharmaceutical representatives for more samples when needed.
OTHER
Calls patients with the results of biopsies as directed by the physician.
Returns phone calls to patients, pharmacies, etc., giving advice or refilling prescriptions as directed by the physician.
Takes photographs as needed.
Treats all patients and other staff members with respect, is polite and professional.
Additional duties assigned by the medical assistant supervisor, practice manager or physician(s).
