Position Overview
The Licensed Practical Nurse (LPN/LVN) is responsible for providing and documenting skilled nursing care, under the supervision of a Registered Nurse, in accordance with the developed care plan and physicians orders for each individual patient while adhering to confidentiality standards and professional boundaries at all times.

This may include the completion of Medication Administration Record (MAR) duplicate sheets for any changes in a particular resident's medication, performing monthly audits/checks on all residents' charts, assisting with the processing of a new resident's paperwork, filing and maintaining resident files in a professional manner.
6. As requested, maintain an inventory of all department supplies. Notify the Health Services Director or Journey Program Director timely and/or place supply orders, with a supervisor's approval, when supply levels reach a pre-designated threshold.
7. As directed, act as the main point of contact for physicians, family members and vendors in the absence of the Health Services Director or Journey Program Director.
8. At the direction of the Health Services Director or Journey Program Director, assist with department scheduling including covering call-offs, vacations and the elimination of overtime.
9. As required, update the Caregiver Monthly Assignment Journal, Medication Technician Med List and 24 Hour Communication Log.
10. Assists residents with their daily medications, treatments and resident care as assigned and within the scope of practice, policy and procedures and Title 22 Regulations.

13. Keep medicine carts and areas orderly, clean and appropriately stocked and locked. Maintain upkeep and cleanliness of medicine carts and medication rooms.

Use only authorized abbreviations established by this facility when recording information.
Report all changes in the resident's condition to the Nurse Supervisor/Charge Nurse as soon as practical.
Record all entries on flow sheets, notes, charts, etc., in an informative, descriptive manner.
Report all accidents and incidents you observe on the shift that they occur.
Agree not to disclose resident's protected health information and promptly report suspected or known violations of such disclosure to the Administrator.
Report any known or suspected unauthorized attempt to access facility's information system.
Perform only those nursing care procedures that you have been trained to do.
Ensure that the resident's room is ready for receiving the resident (i.e., bed made, name tags up, admission kit available, etc.).

Keep residents dry (change gown, clothing and linens, when it becomes wet or soiled).

Job Summary
A Medical Assistant is responsible for preparing participants for exams and procedures; assisting providers with minor surgeries and procedures performed in clinic; taking necessary vital signs and measurements; obtaining and processing lab specimens; scheduling appointments, tests and procedures; assisting providers in any other way to ensure effective participant flow.
Essential Job Functions
Duties include but are not limited to:
• Report on personal care services provided and the self-care abilities of participants, including changes noted in participant's
condition.
• As part of the health care team, provides assistance that will maintain the independence of the participant to conduct
activities of daily living.
• Welcomes participants, visitors, providers and staff in a warm, friendly and professional manner.
• Rooms participants upon arrival, documenting participant's vital signs and chief complaint in the participant's chart.
Confirms current medication list and any additional information as instructed by the provider.
• Initiate all telehealth video and phone visits prior to the provider entering the exam room if needed.
• Assumes responsibility for assisting providers with procedures and tests as directed by the Medical Board of California.
• The day prior to participant's appointment, reviews participant's chart and checks that all pertinent information is up-to-date
including current referrals and test results.
• Reviews charts at the end of each participant day, reconciling orders and review of orders tracking report, and completing
any provider requests.
• Pulls prescription refill requests and obtains approval from provider to refill.
• Calls or faxes in any orders approved by provider.
• Checks voice mail messages regularly throughout the day and addresses/responds to all inquiries by the end of each day.
• Maintains clean work area and cabinets.
• Cleans and sterilizes instruments as part of daily tasks assignment.
• Maintains OSHA logs and keeps up to date for compliance.
• Restocks exam rooms with supplies on a daily basis.
• Inventories and orders medical supplies as needed.
• Checks Inbound Labs or Labcorp link results on a daily basis and notifies provider of participants' lab and test results.
• Remind providers of any reports needed to be turned in and co-signed such as daily orders, HCC report and Pharmacy

• Assure compliance with infection control, personal protection equipment, blood-borne pathogens, hazardous material

• Maintain accurate, up-to date EHR documentation indicating ongoing services provided, reassessments of changing needs,
and participant's expressed wishes.
• Prepare and submit timely reports as required.
