JOB DESCRIPTION:
SUMMARY OF POSITION
Centerstone is seeking a compassionate and dedicated Care Specialist to join our Certified Community Behavioral Health Clinic (CCBHC) team. The ideal candidate will provide high-quality behavioral health services to individuals, families, and groups, focusing on promoting mental health, wellness, and recovery.
A Care Specialist provides person-centered assistance to clients in the development of life skills and personal goals. Coordinates client care with multidisciplinary team. Assists clients in connecting to community resources. Provides crisis intervention/prevention to clients and their families.
ESSENTIAL DUTIES & RESPONSIBILITIES
Coordinates client care by working closely with a multidisciplinary team to develop and maintain essential elements of the client's clinical record, including but not limited to treatment plan, progress notes, reporting and correspondence with external agents, ensuring compliance standards.
Teaches and trains clients in activities of daily living, in group and individual settings, complying with clinical documentation requirements, and provides services according to organizational guidelines and/or protocols.
Connects client with community support systems as determined to be appropriate, and maintains regular communications with schools, OFC's and other family service providers that the client and their family participates with or is receiving services from.
Provides telephone and on-site crisis intervention/prevention and follow-up to clients and their families.
Understands how the job functions within the overall Certified Community Behavioral Health Clinics (CCBHC) model.
Perform all required job duties and responsibilities in accordance to CCBHC Standards.

* Provide community-based mental health services to individuals across the lifespan, including assessments, treatment planning, and intervention implementation.

Qualifications
Education and Experience:

Able to answer patient questions or direct them to the proper resources whether in person or over the phone.

JOB DESCRIPTION:
1st, 2nd, & 3rd Shifts
Responsible for providing face to face, phone, and telehealth crisis response and assessment services to those individuals who are experiencing any mental health crisis. Provides support and linkage to other area resources with follow up services to individuals in crisis and their families. Represents Centerstone in a professional manner when interacting with external entities.
ESSENTIAL DUTIES & RESPONSIBILITIES
Provide coverage of crisis service beyond scheduled work hour/shifts and as assigned by crisis service supervisor.
Maintain accurate records of all contacts and enter all data in a timely manner.
Client's follow-up needs are documented for Team follow-up.
Deescalates crisis situations as needed.
Thorough assessment of risk of harm to self or others is clearly documented and plan to assure safety is thorough and feasible.
Make appropriate referrals for least restrictive treatment possible to ensure psychiatric needs are being met as evidenced by documentation.
Makes every effort to respond to calls promptly.
Creates an atmosphere of cooperation with Centerstone departments, community agencies, mental health consumers, and the public at large.
Attend all team meetings, clinical supervision and completes online trainings as required.
KNOWLEDGE, SKILLS & ABILITIES
Effectively communicate via written, verbal, in person and virtual methods.
Demonstrate knowledge and competency in the utilization of Centerstone programs and other community resources.
Knowledge of crisis intervention skills.

JOB DESCRIPTION:
SUMMARY OF POSITION
Assists Physicians, Physician Assistants, Nurse Practitioners, and Nurses. Prepares examination rooms. Prepares patients for examinations. Processes charts. Assists Office Professionals with clerical duties.
ESSENTIAL DUTIES & RESPONSIBILITIES
Completes and records the following types of tasks in the Centerstone Health Services' EMR:
Patient vital signs
Spirometry readings
Administers EKGs
Assists with office procedures
Administers injections and vaccines
Draws and processes blood and lab specimens
Renders basic first aid when necessary
Ensures that all data elements are captured in each patient visit
Assists physicians, physician assistants, nurse practitioners, and nurses with examinations of patients; assist patients.
Supplies management (ordering and stocking of examination rooms/clinic area); sterilize instruments; clean examination rooms.
Prepares patient information and process related paperwork.
Follows guidelines and procedures set by both the Medical Director and the CHS Quality Assurance (QA) Team.
Responsible for direct patient communications regarding:
Lab results
Future appointments
Hospital admissions
Referral appointments
Able to answer patient questions or direct them to the proper resources whether in person or over the phone.
May be called upon to serve on committees, work groups or other bodies as assigned.
Performs clerical duties, including filing, as needed.
KNOWLEDGE, SKILLS & ABILITIES
Effectively communicate via written, verbal, in person and virtual methods.
Ability to assess and chart accurate information by using equipment listed below.
Ability to provide basic nursing care.
