    Part-Time 0800-1630 Assumes accountability and responsibility for managing the hospitalization of a caseload of patients from pre-admission through one week post-discharge. Facilitates coordination of care by all members of the health care team to facilitate achievement of appropriate outcomes. Responsibilities: 1. Evaluates patient needs, provides case management. 2. In collaboration with the M.D. /CNS, provides direct patient care to patients, including, but not limited to, medication management follow-up and decision-making about follow-up care, crisis stabilization, medication changes, and wraparound services. 3. Assists in the planning and implementation of medical care and coordination of services with other patient care units, both within the hospital and through outside institutions and links families with community resources. 4. Care is established from the time of admission to the service through termination by utilizing the nursing process in such a manner as to maximize outcomes. 5. Additional duties include the development and implementation of materials necessary to facilitate patient care with varied care providers, patient and family education, staff and care provider education, development of patient and family educational materials. 6. Participation in research, basic supportive counseling, procurement of community resources for families, and assessment of changing patient needs throughout the course of care with appropriate referrals for consultation as needed. Other information: Technical Expertise 1. Must possess excellent oral and written skills. 2. Demonstrate leadership and interpersonal relationship skills which enhance communication and promote conflict resolution.
