Job Summary
The Advanced Practice Provider (APP) cares for our patients within the scope of training and approved the agreement, as outlined by their governing board. Works collaboratively with other providers and staff to ensure efficient patient flow and a high level of patient satisfaction. This position works directly with a franchised business location. Responsibilities
* Perform complete, detailed, and accurate health histories, review patient records, develop comprehensive medical assessments, and order laboratory, radiological and diagnostic studies appropriate for complaint, race, sex, and physical condition of the patient
* Formulate medical and nursing diagnoses and institute therapy or referrals of patients to the appropriate health care facilities, agencies, and other resources of the community or physician
* Institute emergency measures and emergency treatment or appropriate stabilization measures in situations such as cardiac arrest, shock, hemorrhage, convulsions, poisoning, and allergic reactions
* Interpret and analyze patient data to determine patient status, patient management and treatment
* Provide instructions and guidance regarding health care and health care promotion to patients/family/ significant others

POSITION OVERVIEW: The Adult Attendant Care/Psychosocial Rehabilitation, Individual Worker is responsible for assisting skill building opportunities with consumer. The goal is for the consumer to become more self-sufficient in daily living skills. This position works one-on-one with consumers to provide crisis support, assistance with the utilization of community resources, assistance with symptom management, and with other activities which promote the recovery of mental health consumers.
ESSENTIAL POSITION RESPONSIBILITIES:
Provides services in order to maintain required productivity/billing standards set by the department.
Meets deadlines and ensures accuracy of various reports/paperwork, mileage records, and electronic timesheets.
Maintains accurate and timely documentation of service provision. Completes progress notes in a manner that individualizes each note, reflecting appropriate interventions and progress towards goals.
Submits required progress notes/billing information in a timely manner as per Agency, Medicaid, and COMCARE requirements.
Provides AC/PRI services in accordance with the consumer's treatment plan goals.
Provides motivation, support and instruction for consumers to achieve self- sufficiency in the activities of daily living necessary to maintain personal stability in their current living arrangements.
Provides support in crisis situations.
Observes and notifies supervisors & treatment team of changes in consumer condition.
Notifies supervisors of a consumer's non-compliance with treatment plan goals or the need to change goals.
Assists consumers with utilizing resources within the community.
Provides consistency in services to consumers by arriving on time to scheduled appointments and/or notifying consumers in the event of tardiness.
Collaborates with the treatment team & Care Manager to ensure better health and appropriateness of services.

The worker must be reliable and responsible, with the ability to respond appropriately to emergency situations and will preferably have experience with the SPMI population. POSITION SUMMARY:
The Direct Support Professional (DSP) is much more than a caregiver. Maintain recordkeeping of PCSP and represent the organization when meeting with case manager, families, and guardians.
Serve as a role model for individuals served, displaying a positive attitude while fostering a positive team environment.
Transport, accompany, and supervise individuals to and from the program, as well as, out into the community to promote engagement and independence.
Comply with all DOT regulations and complete driving logs, checklists, and maintenance records as needed on vehicles utilized for transportation.
Complete incident reports for any accident, illness and/or observations of consumer care.
Responsible for administration of individual's served medication and proper documentation of medication dispensed.
Provide basic personal hygiene assistance and meal preparation for individuals served.
Monitor sustaining activities, projects, and levels of customer service for material handling operations.
Set up and complete time studies as requested by Manager or Supervisor.
Arrange workstations for production work, including set up of workspace, maintaining supply of raw materials, quality checks on items produced and cleanup of work area.

Experience: Previous experience working with developmentally disabled individuals, or CNA/care giver experience required. 2-4 years caregiver experience preferred.

REPORTS TO: Children's Case Management Team Lead
The Children's Respite Worker provides one-on-one service to children with serious emotional, behavioral or psychological disorders, with the purpose of providing relief to families/caregivers. Services can be home, community, and school based.
ESSENTIAL POSITION RESPONSIBILITIES:
1. Provides services in order to maintain required productivity/billing standard set by the department.
2. Meets deadlines and ensures accuracy of various reports/paperwork, mileage sheets, and electronic timesheets.
3. Maintains accurate and timely documentation of service provision. Completes progress notes in a manner that individualizes each note, reflecting appropriate interventions and progress towards goals.
4. Submits required progress notes/billing information in a timely manner as per agency requirements.
5. Provides Respite services to children/families at scheduled times following the plan of care.
6. Provides support/mentoring to child, teaching self responsibility and goal achievement.
7. Participates in community activities with child when possible and/or engages the child in planned activities such as community service.
8. Documents and notifies the program Lead of emergency situations, concerns or incidences.
9. Participates in training sessions to learn how to assist children in daily-living activities, deal with symptoms of mental illness, handle emergency situations appropriately, and provide personal support to consumers.
