Reviews resident Service Plans as needed to gain and maintain a familiarity with resident services needs and preferences
Reviews the Staff Communication log and corresponding resident Service Notes on a daily basis for information about changes in resident service needs and/or health conditions
Provides personal services to residents as assigned and as indicated on resident Service Plans
Helps residents with medications and/or treatments as indicated in the medication book and as assigned and allowed by regulation
Responds to calls from the emergency call system in a timely manner
Demonstrates knowledge of and follows infection control procedures
Helps maintain a clean, comfortable, and safe environment by providing housekeeping services as needed
Helps with individual resident and group social/recreational activities as requested and encourages resident participation in activities
Monitors for changes in resident needs, preferences, and/or health status and reports/documents any changes according to established procedures

Prepare and clean municipal aquatic facilities. Job responsibilities include, but are not limited to: litter control around the facility area, restroom cleaning, chemical checks, system maintenance and keeping general areas around the facility clean and presentable at all times. Position will perform their duties under the direct supervision of the Aquatics Coordinator and Assistant Aquatics Coordinator.
Essential Functions
Prepare and clean the three municipal splash pads. Tasks include:
Perform opening and closing checks daily.
Clean splash pad area, amenities area, and restrooms.
Perform minor repairs needed.
Report major repairs needed.
Report potential safety hazards.
Perform necessary daily or weekly pump house maintenance as needed.
Keep pump house neat and clean.
Check all amenities and keep in good condition.
Water chemistry checks and record findings.
Transport chemicals and other supplies in between splash pads if needed.
Tour each facility each day to check for safety issues or hazards and report them to the proper personnel.
Maintain chemicals and supplies needed to clean and prepare the facility, follow all safety guidelines for handling chemicals.
Maintain equipment used in cleaning and preparing the facility.
Test water chemistry and record measurements
Must be able to provide accurate maintenance reports and log all work done on each of the pools.
Each facility will be deep cleaned at least once a week depending on need basis. All facilities will be checked and spot cleaned each day.
Maintain records and submit any required reports to the supervisor.
Enforce facility rules to public when necessary.
Perform related duties as required.

Responsibilities:
Providing behavior intervention, health, socialization, recreational, and personal care services and supports, outlined in the Individual Program Plan, for individuals receiving services.
Provide assistance in personal hygiene tasks, ranging from total physical assistance to monitoring (including toileting, bathing, grooming, dressing, washing hands, brushing teeth, etc.).
Provide physical assistance as needed, such as using adaptive equipment to/from wheelchair to bed, tub, shower, etc.

StarCare Specialty Health System (StarCare) is seeking a part time Direct Support Professional / Community First Choice Trainer to join its outstanding IDD Team supporting people with intellectual and developmental disabilities in the community.
The Direct Support Professional provides the support and care of adults with disabilities of all ages. The Direct Support Professional embodies and protects the human rights of individuals with intellectual and developmental disabilities and practices person-centered planning which actively supports their full inclusion and participation in the community. Support is provided in the person's home and their community.
Responsibilities:
Providing behavior intervention, health, socialization, recreational, and personal care services and supports, outlined in the Individual Program Plan, for individuals receiving services.
Provide assistance in personal hygiene tasks, ranging from total physical assistance to monitoring (including toileting, bathing, grooming, dressing, washing hands, brushing teeth, etc.).
Provide physical assistance as needed, such as using adaptive equipment to/from wheelchair to bed, tub, shower, etc.
Assist people with eating, such as meal planning, grocery shopping, teaching appropriate table manners and how to order food in a restaurant.
Monitor or assist people with the taking of medications.
Perform and/or assist in housekeeping duties, trash disposal, cleaning/sanitizing bathroom and house, laundry, sorting and folding clothing, and other related tasks.
Provide training and support in a variety of areas, including socialization skills, recreational activities, shopping, community events, etc.
Assist supervisor and other staff with special assignments and/or projects.
Essential Duties:
Complete required documentation of services, including contact notes, mileage logs, service activity logs, time sheets, and other related documentation.
Communicate pertinent information to other trainers, nurses, supervisor, etc. either verbally or in writing.
Schedule: This is a part-time/non-exempt position.

Qualifications:
High School Diploma or equivalent, preferred.
However, three (3) years of Direct Support Professional experience and successful completion of a proficiency assessment will qualify.
