Position Overview
Under the direction of the Resource Coordinator - Home Care, the Home Care Attendant (HCA) functions as a member of the Home Care team and is responsible for the provision of personal care and home support services to an individual or group of home care clients as assigned. In this role you will:

Position Overview

Job Responsibilities
Providing excellent nursing care to post-acute residents in areas nearby
Supporting residents with basic activities of daily living (ADLs) like eating, personal hygiene, and grooming
Protecting resident health and safety by supporting a clean, sanitary, and hazard-free environment
Assisting with the transportation and movement of residents around the facility, as needed
Documenting and reporting resident vital signs and updating the nursing team on the physical and mental conditions of residents
Being accountable to follow and adhere to care plans for residents and all applicable policies, rules, and regulations
Engaging with residents in ways that build relationships based on a sense of security and trust
Communicating between, and among, staff, residents, and family members in ways that support transparency and the comfort of residents
Qualifications and Skills
Current Ohio State Tested Nursing Assistant (STNA) certification
Demonstrated knowledge of nursing care procedures and clinical terminology
Detail-oriented and able to effectively communicate with medical professionals, residents, and family members
Ability to lift a minimum of 50 lbs
