Summary: The Classroom Aide/Driver 3 transports individuals safely to and from day service programs as their Driver. As an Aide, duties include developing, implementing, and supervising a training program designed to meet each individual's needs, case management of designated individuals, and participation in general duties. Schedule: 8:00AM-4:00PM Monday-Friday (40 Hours) Essential Duties and Responsibilities: * Work with and train individuals on specific day service plan objectives. * Responsible for data collection of goals of each individual. * Responsible for daily documentation of medical data or target behaviors. * Participate in all general programmatic duties (arrivals, breaks, lunches, departures). * Assist with the upkeep and maintenance of the program, follow the cleaning schedule, and report any maintenance issues. * Protect each individual's rights and maintain confidentiality of any information relative to people. * Complete any administrative duties as assigned. * Participate in all required meetings. * Maintain all appropriate certifications including FA/CPR and PABC. * Assist all individuals in entering and exiting the vehicle and ensure all individuals have properly secured seatbelts. * Interact with patience and sensitivity in all contact with individuals, family members, staff, and others. * Operate the vehicle at all times within all federal, state, and local laws. * Have good knowledge of routes and locations of all houses, day facilities, and emergency locations. * Maintain and carry emergency fact sheets, daily attendance, and trip route sheets. * Complete a quick vehicle safety checklist before beginning your assigned route and report any issues to your supervisor immediately. * Immediately notify the supervisor of any variance in route time exceeding 15 minutes. * Maintain all required certifications * Other duties as assigned. Required Education/Experience: * A minimum of a High School Diploma or GED is required. * Must be 20.5 years of age or older, with at least 3 years of driving experience. * Must have a valid MA driver's license and a good driving record history with no more than 2 moving violations in the last three years. Required Skills/Knowledge: * Previous experience working with individuals with disabilities is preferred. * Ability to interact with patience and sensitivity in all contact with individuals, residential staff, parents, and facility staff. Required Background Check: * Requires acceptable annual CORI according to Bridgewell policy and a fingerprint-based national criminal background check per DDS regulations and policies. * A Drug Screen and physical must be conducted as soon as possible after a successful interview; regular employment is contingent upon acceptable results according to Bridgewell policy. * An annual driving record history will be verified and results are subject to MART approval based on regulations and annual MART audits Physical Demands: The physical demands described here are representative of those that must be met by an employee to successfully perform the essential functions of this job. Oversee contract relationships to include regular reporting on contract performance as well as new revenue generation and growth. Leads development of continuum of care to include comprehensive outpatient services and community-based programs. Serves as the final authority for resolution of staff performance concerns and performance improvement activities as appropriate. Routinely attends and as appropriate, chairs periodic meetings with the governing body, medical staff, executive management team, JV Board of Directors, and other departments of the facility. Abides by all company policies and procedures and operates with the highest personal integrity and professionalism. Remains current in all national healthcare-based initiatives through participation in such organizations as NAPHS, NABH, and the relevant state-based facility associations. Ensure an effective survey readiness plan is always active including a comprehensive auditing plan, corrective actions taken to address noncompliant areas, and preventative actions to maintain continuous accreditation and regulatory compliance. Ensure adherence to the Corporate Compliance program including timely follow-up with reported compliance issues, staff training, and proactive auditing. Confirm and lead accurate Governing Board reporting and quarterly calls. Ensure appropriate support for QAPI activities including direct and leadership staff resources, training, and other requirements. Summary: The Asleep Overnight CNA 3 will provide support and assistance to individuals with developmental and psychiatric disabilities during the overnight hours as needed in Level 3 residential programs. Staff will provide support to individuals who may need daily intervention, supervision, and skills training in activities of daily living while managing within a home environment and in the community. Individuals may require moderate to considerable physical assistance or accommodations. Staff will receive specialized training to provide enhanced supervision or interventions designed to address multiple disabilities which may include behavioral supports or enhanced medical care. Schedule: Wednesday-Saturday 11:00PM-9:00AM ASLEEP OVERNIGHT (40 hours) Essential Duties and Responsibilities: * Provide support and assist individuals with all activities of daily living during the morning routine which may include assistance with grooming, bathing, oral hygiene, feeding, incontinent care, toileting, colostomy care, prosthetic appliances, transferring, ambulation, range of motion, communicating, or other needs in keeping with the individuals' care requirements. * Provide care during the overnight shift to individuals who may wake up or have needs requiring assistance including checking vital signs, checking their weight, applying creams/ointments, etc. * Provide care to individuals such as maintaining the comfort, privacy, and dignity of each individual in the delivery of services to them. * Maintain safety of all individuals while on shift including implementing emergency policies as needed. * Provide services to individuals according to, DDS, DMH, MassHealth, HUD, DPH, CARF, and all applicable regulatory standards, and Bridgewell policies and procedures. * Maintain daily and monthly progress notes, shift checklists, and other program documentation according to DDS, DMH, and all applicable regulatory standards. * Administer medications according to MAP regulations. * Maintain core training certifications including CPR/FA, OSHA, PABC, and MAP. * Maintain additional mandated training and certifications as required by the program. * Complete household cleaning duties as assigned. * Other duties as assigned. Required Education/Experience: * Must be a high school graduate with a minimum of 1 year of experience working with people with developmental or psychiatric disabilities. * Successful completion of a Massachusetts-approved Nursing Assistant Certification course or other education that meets state certification requirements. * Must hold current Nursing Assistant Certification in Massachusetts and maintain active certified status. * A bachelor's degree may be substituted for a CNA based on experience. Required Skills/Knowledge: * Excellent written and oral communication skills so that the writing of required documents is clear and articulate. * Excellent organizational and problem-solving skills. * Valid driver's license and one year of driving experience. * Computer literacy includes the ability to use proficiently Microsoft products and web-based applications used by the agency. Physical Demands: * While performing the duties of this job, the employee is frequently required to climb stairs. * Must be able to lift to 50 pounds. * The employee must be able to drive a vehicle and regularly meet the driving record requirements of the agency. * Must have a normal range of eyesight with or without corrective lenses. * Must have a normal range of hearing with or without corrective equipment.
