Hospice Journey Counselor
Behavior Therapist Job In New Kensington, PA
AHN Healthcare@Home Hospice team is looking for a Journey Counselor. This position will provide information, education, and support to patients, families, and caregivers regarding end-of-life planning and care. This role will assist them (patients, families, care-givers) in the transition to Hospice care when the patient is both eligible and appropriate for Hospice services.
Journey Counselor Responsibilities:
Manage Journey patient census and referrals for the assigned area from Journey candidate list, Journey Administrative Assistant, Customer Service, Clinicians, and other sources
Review all assigned patient charts to determine eligibility and appropriateness for the Hospice benefit and prepare for conversations with the patient, family, and/or caregivers
Make initial phone calls to all new Journey referrals to schedule home visits as well as follow-up calls to maintain contact with the patient and assess patient decline
Conduct patient visits for the purpose of building relationship with the patient and/or family and in-depth discussion of the hospice benefit when appropriate
Maintain Journey patient charts, documenting all Journey visits and contact in Journey Visit/Phone call notes or as Coordination notes when applicable
Build relationships with and maintain effective communication with Clinicians to ensure effective collaboration for addressing patient needs
Assist clinicians in a more accurate understanding of the philosophy, goals, eligibility, and benefits of Hospice care
Qualification Requirements:
High school diploma/GED required
Bachelors Preferred
Prior home healthcare or relative work experience in the healthcare industry preferred
Understanding of the hospice philosophy preferred

What does a Behavior Therapist do?

Intensive Behavioral Health Services (IBHS) provide behavioral health treatment to children up to age 21 who have a current DSM or ICD mental health diagnosis and require behavioral support. The services use professional staff and natural supports to create a network of support around an identified child and their family. Services can be rendered in a child's home, community or school location dependent upon the child and family's needs. Services include utilizing expertise to help families in implementing an individual treatment plan (ITP) developed by the lead clinician and based on their goals and objectives. The use of community resources to achieve maximum improvement in the child's functioning with minimum disruption to the family unit is key. REQUIREMENTS:
1. Individuals who provide individual services as Behavior Consultant/Mobile Therapist shall meet ONE of the following:
Licensed psychologist, professional counselor, marriage and family therapist or clinical social worker; OR
Licensed social worker or behavior specialist AND a mental health service graduate degree and practicum; OR
Graduate degree from accredited program with at least nine (9) credits in psychology, social work or counseling AND at least one (1) year experience providing mental health services; OR
Completed accredited graduate program with clinical/mental health service practicum with at least nine (9) credits in clinical psychology, social work, education, counseling, or a related field.
2. Must have access to the audio / visual / verbal technology to provide supervision for the clients and to perform essential job functions
3. Must possess the emotional competence to perform essential job functions

You are an enthusiastic and dedicated Registered Behavior Technician who will treat, track, and report the effectiveness of Applied Behavior Analysis (ABA) related interventions for individuals with autism, developmental delays, and other learning disabilities while working with a multidisciplinary team and under the supervision of a Board Certified Behavioral Analyst (BCBA) in a school-based environment.
WHAT YOU'LL DO
Student Supervision - Supervise students in classrooms, halls, cafeterias, school yards, the gross motor room, and on field trips
Treatment - Provide 1:1 behavioral and skill acquisition support and treatment for students in identified areas of need
Data Tracking and Analysis - Ensure accurate and reliable data collection on all IEP goals and other target skills related to ABA curriculum using data sheets and electronic marking devices in order to identify skill acquisitions, behavioral improvement, and mastered target achievement
Reporting - Complete documentation in accordance with Hope deadlines and legal timelines
Collaboration - Collaborate with IEP and treatment team including teachers, schools, and behavioral personnel to maximize daily planning, record keeping, goal achievement, accommodations, and modifications
Training - Actively participate in staff meetings and in-service training programs to continuously update skills
Other responsibilities as assigned.

Position Overview:
We are seeking a compassionate and dedicated Board Certified Behavior Analyst (BCBA) to join our team. Our BCBAs are responsible for creating an environment of trust with the families we serve, designing and overseeing behavior intervention plans specific to individual needs, supervising and mentoring Behavior Assistants, Registered Behavior Technicians (RBT) and Student Analysts, while ensuring high-quality ABA services are provided to clients.
Responsibilities:
Conducting Functional Behavior Assessments (FBAs) and develop individualized treatment plans
Providing supervision and training to team members, including PBS employees assigned, family members, and stakeholders who support the clients success
Implementing and overseeing ABA therapy programs in home, school, or community settings to prioritize behavior reduction, skill acquisition and generalization of learned skills across environments
Collaborate with families, caregivers, and other professionals to support client progress
Analyze data to monitor client outcomes and adjust treatment plans to ensure success of the client and family
Ensure compliance with ethical guidelines set by the BACB and PBS policies
Qualifications:
Must have active BCBA certification and be in good standing with the BACB
Masters degree in Applied Behavior Analysis, Psychology, Education, or a related field
Experience in developing and implementing behavior intervention plans
Strong leadership, communication, and problem-solving skills
Ability to work independently and as part of a team
Reliable transportation and ability to travel to client locations as needed
