Case Manager FACILITY: SAMARITAN BEHAVIORAL HEALTH Case Manager functions as a member of a team to assist in the planning, development, and delivery of outcome-effective treatment services in various types of settings. This position maintains an ongoing working knowledge of agencies and community/emergency resources for child, adolescent, adult, and geriatric clients and uses that knowledge to coordinate services and advocate for identified clients. Case Manager acts as a resource consultant to other team members and collects resource materials regarding other agencies and services in the community. Job duties:

· Collaborate with referral source, internal team members and external service providers to ensure appropriate client care and continuity of care.

· Provide other direct, face to face, services to clients via individual, group, and family therapies according to departmental standards and productivity expectations.
· Provide CPST services to coordinate clinical care in the form of face to face, telephone calls, report writing and case conference meetings.
· Demonstrate knowledge and skills relevant to the service method provided to clients.
· Maintain scheduled appointments and other workload activities on CareLogic schedule according to departmental standards.
· Seek medical provider consultations regarding medical needs of clients. Maintain on-going communication with medical provider for the purpose of coordinating and evaluating client care.
· Demonstrate strong customer satisfaction skills in the provision of clinical care services.
· Demonstrate knowledge of SBHI medical records and CareLogic and other paperwork standards. Apply competencies in both the provision and documentation of medical records.
· Demonstrate compliance with departmental standards for administration and reporting of outcome measures and documentation of utilization management of clinical care.
· Demonstrate knowledge of other regulatory standards. Ensure that clinical records are maintained in compliance with these standards at all times.
· Demonstrate knowledge of IT and Quality Improvement policies and procedures. Apply them to the documentation of clinical care according to medical records and other departmental standards.
· Assess current competencies. Design and implement professional development plan to improve competencies in targeted goal areas to support departmental strategies.
· Provide documentation of continuing education required for maintaining current professional licensure and competency credentials.
· Participate in assigned programs and staff meetings.
· Meet regularly with clinical director and as needed to review clinical care of patients according to licensing and agency standards.
· Ensure all annual updates are completed in CareLogic for caseload.
· Complete face-to-face CPST assessments within (5) business days after receiving referral.
· Maintain caseload list reflective of current and active clients. Review closure / discharge if no services within 90 days.
· Two year degree in a relevant social science field or equivalent education and experience
· Minimum of two (2) years' experience in the mental health and/or drug and alcohol field.
· Working knowledge of mental health, drug and alcohol, and other community agencies and resources essential.
· Working knowledge of The Diagnostic and Statistical Manual of Mental Disorders (DSM- 5 or subsequent revisions), as related to coordination of community support services.

Monitor effectiveness of the BSI Plan and revise and/or update accordingly based on observation, implemented interventions, and review of data collection forms.
Participate in the development of the Behavior Support Plan with the Behavior Consultant.
Implement the Behavior Support Plan.
Collect and analyze data for the effectiveness of the Behavior Support Plan as approved by the Behavior Consultant.
Monitor fidelity of implementation of the Behavior Support Plan and reliability of the data.

Under the general supervision of the building principal and OEC Academic Coordinator- BCBA, Registered Behavior Technician Paraprofessionals are to assist in the education and learning of our students with behavioral and social emotional needs. An incumbent in this classification will evaluate, plan, and implement activities to remediate and compensate for difficulties in social emotional learning skills such as self-awareness, self-management, social awareness, relationships and responsible decision making that interfere with a student's school performance.
ESSENTIAL DUTIES AND RESPONSIBILITIES

Implement behavior intervention and/or behavior intervention plans as written using sound judgment and seeking of appropriate consultation from BCBA as needed
Accurately and systematically collects data for behavior intervention and behavior intervention planning
Participate in required supervision with Academic Coordinator- BCBA
Establishes and maintains a positive relationship with students and staff
Prompt safe and socially acceptable replacement behaviors including but not limited to communication, social interactions, and problem solving skills
Educates others on the principles of reinforcement under the supervision of the BCAB
Collaborates with school based team and family to provide effective treatment within the role and scope of the behavior technician and guidance from the Academic Coordinator BCBA
Providing direct client care in one-on-one, small group and large group settings
Following the behavior reduction and behavioral skill acquisition protocols
Collecting and recording data based on client behavior
Communicating with clients, parents, and caregivers regarding client progress under the supervision of the BCBA
Assisting behavior analysts with behavior reduction assessments and skills acquisition
Assisting behavior analysts in the preparation of client materials
Under the general supervision of the Classroom Teacher, assist students with special needs in all aspects of the classroom instruction to maximize inclusion and learning.
Assist students with physical or mental disabilities with daily activities
Assist students with toilet and/or changing needs.
Assist students with feeding needs
Participate in the IEP (Individualized Education Program) process and serve as a resource for the student personnel evaluation team as needed
May be assigned as a one-on-one paraprofessional for a specific student. Will assist the student with daily needs and support the classroom as needed
Follows all applicable rules, procedures and regulations governing the proper manner of assistance for all students, including those with disabilities or other special needs
Maintain certification in verbal de-escalation and crisis intervention
Maintain Registered Behavior Technician certification per Behavior Analyst Certification Board

We are seeking a Behavior Technician for the 2024 - 2025 school year and beyond to join our team at several of our elementary buildings.
Job Summary: The Behavior Technician is responsible for directly implementing behavior intervention strategies, assisting classroom staff in the implementation of individual student Behavior Intervention Plans, and collecting data to support Functional Behavior Assessments and progress monitoring the Behavior Intervention Plans.
Duties and Responsibilities:
* Confers with teacher and/or appropriate personnel for the purpose of assisting in the evaluation of students' progress and/or implementing students' IEP objectives and/or behavior intervention plan.
* Collaborates with the teacher to adapt work, homework, and individual student schedules to increase desired behaviors and decrease undesired behaviors.
* Assists teachers and parents for the purpose of implementing lesson plans and/or developing students' daily living and behavioral skills.
* Assists student as necessary and appropriate to maximize participation in Art, Music, PE and all academic subject objectives.
* Attends class field trips, community outings, volunteer experiences and work study activities to maximize student participation.
* Monitors individual students, classroom and/or playground activities for the purpose of maintaining a safe and positive learning environment.
* Assists students with boarding and unloading from the school bus.
* Rides transportation with students to & from school if necessary to effectively implement the Behavior Intervention Plan and to ensure safety of student & staff.
* Maintains classroom equipment, students' files/records for the purpose of ensuring a safe and appropriate learning environment.
* Provides assistance to student with toileting, feeding, building mobility and dressing needs including providing services to a handicapped/disabled student which may be medical in nature, and which may be invasive, when expert medical opinion states that it is not required that such services be provided by a physician, registered nurse or licensed practical person but can be provided by a properly trained person.
* Performs record keeping and basic clerical functions (scheduling, copying…) for the purpose of supporting the teacher in maintaining student files and providing classroom materials.
* Responds to emergency situations (injured student, fights…) for the purpose of resolving immediate safety concerns.
* Implements de-escalation strategies with students, paraprofessionals, and other staff in the event of a behavior escalation.
* Attends related workshops and in-service training sessions.
* Performs other duties as assigned by teacher and/or administrator.

Education and Experience:
* High School diploma required.
* Valid certificate / license as issued by the Ohio Department of Education.
* Successful completion of a 40 hour online RBT course.

* Ability to establish and maintain effective working relationships with students, peers, parents and the community.

What You Will Be Doing
We want people to join our team who share our values of love, excellence, respect, empowerment, and well-being. The ideal candidate will provide in-home and community-based respite services and behavioral health services for children, adolescents, and young adults with developmental disabilities and behavioral health challenges. The Behavior Technician will work with individuals and families to provide person-centered care and treatment services.
Minimum Education/Qualifications
Must be at least 18 years of age.
Three years experience working with people with intellectual and/or developmental disabilities or behavioral health challenges OR a Bachelor's degree in a related field.
Must be able to communicate in English, including both spoken and in writing.
Valid Ohio Driver's License with Ohio Bureau of Motor Vehicles; proof of auto insurance; insurable driving record (five points or fewer).
Within 60 days of hire, successful completion of the required training(s) and certification(s) applicable to the assigned program.
Preferred Education/Qualifications
Experience working as a Qualified Behavioral Health Specialist (QBHS).
Required Licensure/Certification
Ability to obtain and maintain First Aid, and CPR - adult, child, and infant. (Training will be provided, if not already certified).
Ability to obtain and maintain Ukeru and medication administration certification (Training will be provided).
