Responsible for developing positive relationships by providing guidance and facilitating interactions and services for children and their families. This position will also provide targeted case management services to children with serious emotional problems, their families; and caretakers and assist clients in maintaining their highest level of functioning in the environment, which best meet, their needs. Services include but are not limited to: medical, social, legal, educational and other support services. Must be willing to travel throughout Volusia and Flagler County. Maintains appropriate documentation for each client; evaluates client's home and school environment and interaction; arranges treatment services for clients as per treatment plan; acts as liaison to special projects. - Bachelor's Degree from an accredited university or college with a major in counseling, social work, psychology, criminal justice, nursing, rehabilitation, special education, health education, or other related human services field preferred; a minimum of one year of full time or equivalent experience working with the target population preferred; a Bachelor's Degree from an accredited university or college in a discipline other than previously noted will be considered if the candidate has three years of full time or equivalent experience working with the target population.
