PRINCIPLE RESPONSIBILITIES AND DUTIES RELATED TO PATIENT CARE:Patient Assessment/Care Planning Counseling: As a member of the interdisciplinary team, assesses patients' psychosocial status, strengths and areas of need that may affect rehabilitation and optimal treatment outcomes as part of the comprehensive patient assessment. Participates in care planning in collaboration with the patient and healthcare team to identify effective interventions that will help the patient meet rehabilitation, treatment goals, and improve quality of life. Utilizes FMS Ultra Care patient education programs, established social work theory and methods and quality of life measurement instruments as part of assessment and care planning to resolve barriers and meet patient treatment goals. Provides established outcomes driven psycho-educational counseling methods aimed at improving areas of need, treatment adherence, vocational/educational rehabilitation and/or quality of life. Provides supportive and goal directed counseling to patients who are seeking transplant. Assesses patient awareness of advance directives; assists with accessing advance directive forms/information and facilitates discussion of advance directive wishes, if necessary, with the healthcare team and the patient's family/support persons. Will provide general information about Do Not Resuscitate Orders and Advanced Directives. Provides information and assists the team and patient with referral to community resources (home health services, vocational rehabilitation, etc.) to facilitate optimal treatment outcomes. Provides usual care and/or Social Work Intensive to address non-adherence and quality of life concerns for all patients. Knowledgeable of and adheres to FMCNA Social Work Policy, including documentation. Patient Education: Assess patient knowledge of kidney disease for barriers that may affect adherence to treatment. Works with patient, family and health care team to provide education tailored to the patient's learning style and needs. With other members of the interdisciplinary team, provides appropriate information about all treatment modalities. Facilitates the transplant referral process and collaborates with interdisciplinary team, if delegated by Clinical Manager. Provides ongoing education to patient/family regarding psychosocial issues related to End Stage Renal Disease (ESRD) and all support services that are available. Reviewed patient rights and responsibilities, grievance information (company and network) and other facilities policies with patient and/or the patients' representative to ensure patients' understanding of the rights and expectations of them. Collaborates with the team on appropriate QAI activities. Insurance and Financial Assistance: Maintains current knowledge regarding local vocational/educational rehabilitation programs and assist patients with referral and access to vocational rehabilitation to enable them to remain employed, become employed or receive education. Understands FMCNA financial policies and procedures as they related to patient services. Collaborate with Financial Coordinators to educate patients about programs that may be available to assist with uncovered services, how to resolve billing concerns, and understand financial responsibilities. Staff Related: Works with the administrative support staff to maintain updated patient resource lists (e.g. maintain updated list of transportation resources). Develops and provides training to staff pertaining to psychosocial topics as needed. Trains staff in Ultra Care Programs as needed. Other duties as assigned. Description Elite ABA Therapy, LLC is an outpatient behavioral health center that provides one-on-one center-based Intensive Behavioral Health Services (IBHS) to children between the ages of 3 to 12 who have been diagnosed with Autism and/or other developmental delays. We are located in Wilkes-Barre Twp, PA and operate year-round. Our clinic hours are Monday through Friday from 9:00 am to 4:30 pm. We are closed on all federal holidays. Elite ABA Therapy, LLC specializes in providing Applied Behavior Analysis (ABA) services, which are the most widely known evidence-based practices for teaching new skills and improving behavior. Our goal is to optimize our clients' independence and promote social integration by teaching meaningful and socially significant skills in the least restrictive environment. Our clinic is designed to simulate an Early Intervention-like setting, but without academic curriculum. Teaching is completely individualized and based on the client's needs identified in their treatment plan. Clients receive 1:1 individual services but are grouped with up to 7 other children to simulate a classroom. Each group of children follow a daily schedule of activities that include both individual and group formats. Individual activities include direct instruction of skill acquisition programs specified in the client's individualized treatment plan, such as teaching communication skills. Group activities include circle time, structured play groups, and planned activities (music, arts and crafts, etc.) to work on social skills and generalizing learned skills to group settings. The daily schedule is designed for 3 different sessions per day; the morning session from 9:00 am to 12:00 pm, the second session from 12:30 pm to 3:30 pm., and the after-school session from 3:30 pm to 4:30 pm. BCBA Service Role and Responsibilities: Board-Certified Behavior Analysts and assistants will be responsible for clinical operations that include conducting RBT trainings, competency assessments, supervision, conducting client functional behavior assessments, writing treatment plans, submitting plans for insurance approval, setting up client treatment goals in practice management software, conducting norm-referenced assessments (VB-MAPP), standardized assessments (Vineland Adaptive Behavior Scales and others), designing and updating individualized DTT programs for each client, training RBT's on implementation of the DTT program, reviewing client data, reviewing RBT session notes, writing daily session notes for billable sessions, and conducting monthly caregiver training (can be done during the client's regularly scheduled session at the clinic or in the home setting). BRIEF In accordance with the needs of the ages and of the population served by us, the BCBA-1/ LBS will organize and maintain data collection and statistical analysis systems for programs; conduct behavioral assessments and program implementation; direct clinical activities and staff/parent training; act as the Behavior Analyst on your assigned cases. Monitor and adjust treatment plans based on data and report progress towards treatment goals at a minimum every 6 months or when the child has met their goals. Conduct VB MAPP assessments, ABLLS, SKILLS, and any other assessments that may be appropriate to the development of a behavior program Review graphs and make intervention changes when appropriate, at a minimum every 2 weeks. Coordinate staff schedules that are working on your cases. Supervise the direct therapists on your specific cases at least 1 time a week for a minimum of 5% of their hours. Follow direction from Clinical Director or Senior Behavior Analyst regarding RBT supervision. Promptly report any problems on the case(s) to the Executive Director, Clinical Director or Senior Behavior Analyst. Report to child line with any suspected abuse within 24 hours of the report or incident. Update assessments every year or more frequently, if required. Complete quarterly progress reports and evaluate client progress towards treatment goals. Update insurance client treatment plans to reflect monthly progress to be kept in binder until end of treatment plan. Original plan with progress records should be turned into the office. Attend treatment meetings and IEP meetings as required. Modal treatment protocols to staff. Treatment Plans, FBA, BIP, and progress updates will be submitted with a 48 hour turn around. Develop and oversee transition/discharge plan. STANDARD REQUIREMENTS Must have own transportation. PHYSICAL REQUIREMENTS Must be able to assume a variety of postures (kneeling, squatting, crouching, sitting, standing) in the course of working with developmental disabilities and severe behavior problems. Must be able to lift and carry clients in braces and other adaptive equipment. Must be able to restrain/hold/transport clients in the course of behavioral management. Must be able to receive detailed oral communication and communicate orally to parents, supervisors and staff. Must have visual acuity to determine the accuracy, neatness, and thoroughness of the work assigned. Oversee case related duties such as protocol dissemination, protocol reviews, and parent/staff training. Utilize safe and appropriate handling, management and guidance procedures when transporting and working with clients. Provide indirect client care, following the prescribed behavioral/protocols/programs. Shadow/supervise direct therapists a minimum of 5% of direct therapist's hours per week. REPORTING RELATIONSHIP Responsibility of level one BCBA to respond to parent concerns, direct therapy concerns, scheduling concerns and case specific concerns within 24 hours. After responding to concerns stated above, if additional support is needed contact a Senior Behavior Analyst. Any concerns regarding direct therapist case re-assignment will be communicated from office staff to supervisor who will communicate any changes to direct therapists and parents. Report to Human Resources on employee related issues. Report to Administrative Director regarding billing, technology and schedule related inquiries. QUALIFICATIONS BCBA at the Master's Level or doctorate level with current certification; Maintain certification of your BCBA and LBS. Maintain CAQH and credentialing with insurance companies. Previous experience working in a reputable clinic, home, or school program is highly desirable. A minimum of 1 year clinical/home/school experience providing direct services to children with Autism, training parents, and supervising staff in the implementation of intervention protocols.
