Come join our Integrated Behavioral Health Department at Greenfield's Medical Practice.Full-time position with a robust benefit package.
Under the direction of the Practice Manager and Chief of Child Psychiatry, the Psychologist or LICSW provides clinical services to patients, couples, and families being treated in the Outpatient Behavioral Health. Demonstrates doctorate level knowledge of developmental stages, personality structure and development, mental health diagnoses, current treatment modalities, and psychological testing methods.
Clinical responsibilities include conducting intake interviews to obtain patient history information, contributing diagnostic information, making preliminary diagnoses, and participating in the development of an interdisciplinary assessment and treatment plan, which will typically include pediatric developmental input, psychological testing, individual insight-oriented or interactive psychotherapy, family therapy, and collaboration with outside agencies (e.g., public and private schools, daycare centers, mental health agencies, child advocacy groups), and contributions by other psychological disciplines.
Provide support to various workgroups, councils, committees, and boards.
Responsibilities
1) Provides developmentally informed and culturally sensitive treatment to patients and families.
2) Evaluates patients to assess presenting problem, contributing history, and current mental status
3) Develops behaviorally specific treatment plans, including measurable treatment goals
4) Completes all documentation related to treatment in the clinical record according to established standards consistent with NCQA guidelines
5) Manages the necessary documentation and provides the information required by third-party payers to secure reimbursement.
6) Conducts meetings with families, significant others, and other service providers as delineated in the treatment plan.
7) Provides psychological testing as requested or necessary for assessment or treatment purposes
8) Provides strategic information and input to the Practice Manager for service quality evaluation and program development.
9) As supported by the Practice Manager or Chief Child Psychiatry, participates in teaching activities and academic activities, which may include the clinical supervision of practicum students, post-doctoral students, and other clinicians as requested.
10) Participates in other special activities of the department as requested by the Practice Manager.
11) Maintain and lead an active behavioral health team to collaboratively care for patients with Clinicians, Residents, and Faculty.
12) Oversee scheduling of behavioral health providers in collaboration with the Residency scheduling team to provide access to patients and educational opportunities for Resident Physicians and other professional students.
13) Oversee the development and implementation of a longitudinal integrated behavioral medicine curriculum to ensure compliance with all ACGME requirements inclusive of education specific to diagnosis, management, and coordination of care for common mental illnesses and behavioral issues in patients of all ages
14) Video-precepting/Direct Observations of Care of residents with feedback, evaluation, and monitoring of clinical growth
15) Education on identification and support of the impaired or suicidal healthcare team member
16) Observes all health and safety requirements.
17) Incumbent is expected to perform other similar and related duties as assigned.

SUMMARY
Functions as an independent behavioral health clinician and provides diagnostic evaluation of patients; provides crisis intervention, individual, group or family psychotherapy appropriate to the patient's problem. Coordinates care with and provides consultation to other behavioral health, medical and community providers. Participates in multidisciplinary treatment teams, develops treatment plans and reviews treatment plans regularly.
EDUCATION/LICENSES/CERTIFICATIONS
Master's degree in social work. Current LICSW. American Heart Association Basic Life Support (BLS) required. Advanced Cardiac Life Support (ACLS) may be required based on specialty.
EXPERIENCE
Three years progressively autonomous experience with group, individual and family therapy required.

The Division of Women's Health at Brigham Women's Hospital seeks an experienced part-time or full-time women's health practitioner for the menopause consultative practice. This is an exciting opportunity to promote the expansion of menopause services within the Brigham Health family. The ideal candidate will be board certified in Internal Medicine or Family Medicine, have a background in women's health, with experience in the management and treatment of menopausal symptoms, and be a Menopause Society Certified Menopause Practitioner or be willing to become certified. The position could also be combined with a research component.
The clinical mission for the Division of Women's Health is to deliver innovative, personalized, high-quality clinical care for women. The Division's clinical site is the Fish Center for Women's Health located at 850 Boylston, Chestnut Hill. The selected candidate will be instrumental in delivering an integrated model of care for menopause patients at the Menopause and Midlife Center at the Fish Center.
Applicants should possess an MD or MD, PhD, and a strong record of clinical experience and leadership to join our interdisciplinary team. Outpatient Clinician (BMC Immigrant Health Center)

Provider will be full time in the Immigrant and Refugee Health Center providing psychotherapy and group support for patients.
The Immigrant and Refugee Health Center at Boston Medical Center is seeking an Outpatient Clinician to provide direct services within our behavioral health program. The clinician will be based at the Boston Center for Refugee Health and Human Rights (BCRHHR) where they will conduct diagnostic evaluations, develop treatment plans, provide psychotherapy (individual and group) to immigrant and refugee patients, and participate in a multi-disciplinary team. The majority of patients served are survivors of torture, and nearly all patients have experienced pre or peri-migration trauma, necessitating that this clinician has strong training in evidence-based psychotherapies for treatment of PTSD. The clinician will also learn to conduct psychological evaluations for asylum cases and develop psychological affidavits. They may also provide testimony when deemed appropriate. The clinician will need to partner closely with behavioral health, case management, medical, and legal navigation team members and participate in basic care coordination for patients. The clinician will also act as a liaison with community partners including immigration attorneys and case managers at local resettlement agencies. The ideal candidate will center cultural humility in their clinical practice, be self-reflective, flexible, intellectually curious, and collaborative. Clinicians with lived migration experience and multilingual capacity (particularly in Haitian Creole and/or Spanish) are encouraged to apply.
ESSENTIAL RESPONSIBILITIES / DUTIES:
· Provides comprehensive psychiatric evaluations in coordination with patient, family, and interdisciplinary healthcare teams, and develops treatment plans based on patient needs/goals.

GENERAL JOB SUMMARY
The Social Worker is an integral part of the Bridge Clinic, a transitional outpatient clinic for addiction care at MGH. This position is a critical member of a collaborative care team that includes a resource specialist, recovery coach, physicians, nurse practitioners, psychologists, nurses, and patient service coordinators.

Job Summary
GENERAL JOB SUMMARY
The Social Worker is an integral part of the Bridge Clinic, a transitional outpatient clinic for addiction care at MGH. This position is a critical member of a collaborative care team that includes a resource specialist, recovery coach, physicians, nurse practitioners, psychologists, nurses, and patient service coordinators.
This position will both provide direct clinical care and associated team-based care coordination. The primary clinical responsibilities will be to provide psychosocial assessment, treatment planning, brief treatment and team consultation, for Bridge Clinic patients, particularly those with co-occurring mental health and substance use disorders.
This role will also have the responsibility to assist with care coordination for patients with severe dual diagnosis conditions. Specifically, this role will provide brief SUD counseling with expertise in assessment, motivational intervention, CBT, and psychoeducation.
In addition, this role will collaborate with all team members to develop and support transition plans for these patients within all levels of the addiction treatment care and mental health continuum.
PRINCIPAL DUTIES AND RESPONSIBILITIES
Intake Coordination:
- Assess patients who are referred to Bridge Clinic from inpatient, ED, and community settings with complex mental health and addiction needs to aid with evaluation, treatment planning, and linkage to most appropriate care placement.
- Assist in providing brief assessment for urgent or unscheduled walk-in patients to ascertain specific mental health care needs, in collaboration with nurses or provider, and assist with developing effective care plans as needed.
Clinical Care:
- Provide crisis intervention as needed for patients who present for care.
- Provide trauma informed, short-term, outpatient individual and/or group counseling, utilizing psychoeducation, motivational interviewing, and cognitive behavioral therapy techniques to assist patients with treatment readiness, recovery stabilization and wellness goals.
- Provide clinical support to complex patients on transition planning as they may need clinically informed transfer of care with focus on motivational interviewing and trauma-based perspective.
Care Coordination:
- Work with the Bridge Clinic leadership team to develop best practices for patient referrals to behavioral health services in the clinic.
- Collaborate with team members to cultivate relationships with external behavioral health services to successfully transition patients in need of ongoing care when appropriate.
- In collaboration with Resource Specialist, act as initial point person to communicate with outside treaters (e.g hospitals, CSS) about care coordination for active Bridge patients with dual diagnosis either verbally or electronically, as well as those transitioning to community programs.
- Ensure that all new patients to Bridge Clinic are introduced to the model of the clinic and begin working on a transition plan when medically appropriate.
Work collaboratively with patients and Bridge Clinic staff to assist with patients' transition to long term community-based addiction treatment as appropriate. Ensure integration of care and effective utilization of resources through review of documentation, discussion with providers, and collaboration with patients.
- Support patients' transfer to all levels of care and interactions with other systems (DCF, probation, incarceration etc.) through the provision of clinical information and documentation to other facilities and systems. This may include drafting letter of engagement and preparing information to fax, copying portions of the medical record, obtaining releases of information, and engaging in collateral contact.
- Collaborate with team leadership as appropriate to optimize co-occurring mental health and SUD care, including connection to further supports as clinically indicated.
- Complete follow up outreach to patients or treatment organizations as needed to ensure a successful transition to the treatment organization in the disposition plan.

PRINCIPAL DUTIES AND RESPONSIBILITIES
Behavioral Health Lead
• Provide leadership for the team of Behavioral Health clinicians, including social workers, psychologists and psychiatrists operating in the Bridge Clinic.
• In consultation with Psychiatrist and Medical Director, provide guidance on care for patients with complex behavioral health issues to other Bridge Clinic staff, including providers and administrative staff.
• Provide guidance on specific behavioral health treatment models and workflows for caring for Bridge Clinic patients.
• Meet with behavioral health providers in the clinic (i.e.: psychiatrist and psychologists) at a regular interval to understand and address any challenges to effectively providing services in the clinic.

Under the direction of the Director of Behavioral Health, Nurse Manager or Program Supervisor (as noted above), the Behavioral Health Clinician II (“BHC-II”) in the psychiatric inpatient unit is responsible for performing a wide range of duties involving patient evaluation, case management, treatment, discharge planning, and documentation. The BHC-II is responsible for gathering a psychosocial evaluation through interviews and record review, and completing a written assessment with case formulation and treatment plan.
