Responsibilities
Here's what to expect as a Specialist:
Establish and sustain a productive relationship with parents and guardians of Agency with Choice individuals, and the designated Managing Employer and Support Service Professional(s) of each individual.
Develop and enhance partnerships with public and community agencies.
Meet in person with the Managing Employer to review satisfaction and compliance with the Agency with Choice services, reviewing supports provided and completion of all required annual paperwork.
Attend all meetings including Individual Support Plan meetings and any additional team meetings to support the individual to ensure services are delivered appropriately.
Update daily activity logs and transportation Logs with specific individual outcomes; including the distribution of required forms to Managing Employers and Support Service Professionals.
Provide re-education training and on-going training for individuals, Managing Employers and Support Service Professionals.
Manage and audit all daily activity logs by reviewing for electronic visit verification accuracy, auditing for over utilization, and ensuring proper service documentation. Communicate with the Managing Employer if over utilization occurs and with the Support Service Professional if additional documentation is needed.
Record all communications to and from Managing Employers, Support Service Professionals, and all other team members regarding Agency with Choice services.
Qualifications
