Job Details Since 2013, the Autism Academy for Education and Development (AAED) has opened six campuses in the Metro Phoenix and Tucson areas including our online campus. Responsibilities Assist students with daily behavior management, living skills, and academic lessons. Teamwork in the classroom to support academic instruction and implement behavior plans. Implement lessons as directed by teachers, assist with independent work, projects, and lead small group lessons. Provide behavior modification in alignment with Applied Behavior Analysis (ABA) as directed by our Behavioral Team and BCBA. Participate in ongoing training to further develop skills and working knowledge. Maintain professional conduct and appearance. Maintain compliance with all company policies and procedures Communicate effectively and professionally with co-workers, families, businesses, and the public at large using exemplary written, verbal, and listening skills. Demonstrate conflict management skills by actively listening, finding solutions to problems, and working well with difficult people. Perform related duties as assigned by supervisor. Job Details Entry HOPE Group Main Office - Phoenix, AZ Full-Time/Part-Time High School Any Entry LevelDescription Direct Service Providers deliver Habilitation and/or Respite services to eligible HOPE Group clients who have Autism, Developmental Disabilities, and/or other related disorders. The primary responsibility of this position is to provide short term relief services to parents/guardians while maintaining the safety and well-being of the individual receiving services. Direct Service Providers may be responsible for the direct implementation of individual support plan goals, which address client specific needs. This position requires you support clients in individualized daily living and self-help needs (including but not limited too); behavior, socialization, communication, memory, organization, mobility, motor skills, etc. ESSENTIAL DUTIES AND RESPONSIBILITIES Provide services (Habilitation and/or Respite) as identified in each clients Individual Service Plan, which can include, bathing/toileting, meal preparation, grooming/dressing, companionship, etc. Help clients increase independence at home and in the community (outlined in the Individual Service Plan). Complete all appropriate documentation such as data collection notes, individual work records, and other work-related documents. Complete all time sheets, leave slips, accident/incident reports, etc., within their stated due dates. Use professional language and judgement while working with client's, families, and staff through the Division of Developmental Disabilities (DDD). Maintain all required work certifications and pass DDD required training. Maintain thorough communication with the HOPE Group staff, as well as families served. Ensure a healthy and clean work environment. KNOWLEDGE, SKILLS & ABILITIES Minimum education; high school diploma or equivalent. Basic computer skills (ability to clock in and out of an electronic device). Ability to define behaviors, collect data, and understand client needs. QUALIFICATIONS & POSITION REQUIREMENTS Valid State ID; must be at least 18 years of age. Successfully complete HOPE Group Orientation. CPR and First aid certification. Proof of training and proficiency in A.A.C. Title 6, Chapter 6, Article 9. Fingerprint Clearance Card (FPCC). Successfully pass criminal background check. Reliable transportation. Three (3) verifiable references. A Certified Occupational Therapy Assistant (COTA) works under the supervision of a licensed Occupational Therapist (OT) to assist in providing rehabilitation services to individuals with physical, mental, or developmental conditions that affect their ability to perform everyday activities. The COTA helps patients improve motor skills, cognitive abilities, and daily living tasks to enhance their independence and quality of life. Documentation and Record Keeping: Document patient progress, goals, and treatment results in compliance with healthcare regulations and organizational policies. Complete progress notes and assist in the preparation of reports, maintaining up-to-date patient records. Ensure all documentation complies with HIPAA and confidentiality standards. Collaboration with Healthcare Team: Communicate regularly with the supervising Occupational Therapist, physicians, nurses, and other healthcare team members to ensure coordinated care. Provide input on patient progress and assist in the development of treatment plans in collaboration with the OT. Work as part of a multidisciplinary team, contributing to patient care discussions and planning. Equipment and Facility Maintenance: Maintain and clean therapy equipment and adaptive tools used during treatment sessions. Ensure the therapy space is organized and safe for patients and staff. Requirements Under the general direction of the Direct Service Manager, a Direct Service Provider is responsible for providing support, guidance, and direction to members attending Community or Center-Based programs. Direct Care Providers offer MIKID licensable services to primary caregivers while supporting the emotional, physical, and mental well-being of members and their families. Services may be provided in the child's home, a licensed facility, or in the community. Candidates for this position should have a High School degree or equivalent and be at least 21 years old in accordance with medical licensing regulations. Additionally, one year of experience in behavioral health or childcare is required. Effective written and verbal communication skills, along with excellent organizational abilities, are essential. Preference is given to candidates with prior experience in the behavioral health field and those who demonstrate bilingualism. Candidates should also possess CPR/First Aid Certification and a current Arizona Fingerprint Clearance or have the ability to obtain one. A valid Arizona Driver's License, business use vehicle insurance, and vehicle registration are necessary. Completion of all necessary paperwork and mandatory training prior to providing services is also required.
