Title: Care Manager Aide Department: Social Services FLSA Status: Non-Exempt Reports To: Supervising Care Manager Summary: Responsible for administrating and assisting the MSSP Team in the care management process under the supervision of the Care Manager to frail seniors to avoid premature placement of persons in nursing facilities, while fostering independent living in the community.
