The resident will be involved with formulary and policy development, Medication Therapy Management and population health management across all lines of business, pharmaco-economics/management and clinical pharmacy services for medical home/enhanced primary care sites. The CDPHP pharmacy residency program is a one year post-graduate training experience and is consistent with the requirements set forth by ASHP and AMCP.
