The Peritoneal Dialysis (PD) and Home Hemodialysis (HHD) Registered Nurse provides home hemodialysis care and peritoneal dialysis treatment, including developing initial and ongoing assessments, and providing patient training and education as applicable. The PD and HHD Registered Nurse provides safe, effective delivery of care under the oversight of the Medical Director, Facility Administrator, Senior Director of Outpatient Dialysis, and Vice President of Operations. He/she must comply with the standards outlined in the policy and procedure manual, as well as the regulations set forth by the company and any applicable state and federal agencies. Will require daily travel up to 50% Responsibilities: Demonstrates high level of knowledge and experience skills and competencies related to the Hemodialysis Registered Nurse. Demonstrates ability to perform technical skills for the PD and HHD Registered Nurse including the patient's treatment, water treatment and preparation, and emergency procedures as determined by the competency assessment program. Delivers care to assigned patients as prescribed, including all aspects of the PD and HHD procedure in accordance with state requirements. Initiates, monitors, and discontinues dialysis treatments according to established procedures. Dialyzes isolation patients utilizing isolation precautions and prescribed procedures. Assesses patients utilizing isolation precautions and prescribed procedures. Performs hemodialysis access care according to established procedures. Administers and documents medications as authorized and documented in patient's health record by the physician and further authorized or limited by the State Board of Nursing Practice. Reports any significant information or change in patient's condition to the Clinical Coordinator (CC) or Facility Administrator. Documents clearly, completely, and accurately all pertinent information during downtime of the electronic record. Participates in the formulation, update, and revision of nursing care plans, including long and short-term goals, according to established guidelines. Performs or assists in the performance of all emergency operational procedures, including the evacuation of patients in the event of a fire and/or ordered mock drills. Assists with and performs all procedures of safe operation, disinfections, and inventory control according to corporate and departmental policy, manufacturer recommendations and regulatory agency guidelines. Assists in ordering supplies and maintaining supply inventory and records in an efficient and cost-effective manner. Assists in maintaining neat, orderly, and well-stocked unit. Utilizes equipment, time and supplies economically. Maintains competency in operations, sanitization, maintenance, and of all equipment used for dialysis, water treatment and special procedures. Position Summary: Breakthrough Montessori PCS seeks dedicated aides for the 2024-25 school year who are dedicated to public Montessori and excited to help shape this transformational school. The dedicated aide assists a student, helps them adapt to their classroom environment, and supports their learning to function independently at school. Under the direction of the Special Education Coordinator, and with the support of the Montessori Support Specialists, the dedicated aide uses research-based interventions to support their student's academic, social-emotional, and behavioral growth. Essential Functions and Responsibilities: The Dedicated Aide is a 10-month position. Typical working hours are Mondays from 8:00 am-4:30 pm and Tuesday-Friday 8:00am-4:00pm. Responsibilities include the following: Work with up to 4 students simultaneously while in the resource room; work with up to 2 students simultaneously while in the general education setting Use the research-based strategies or interventions specified in the student's IEP, 504, BIP or other written plan to: Academic Support: Implement and document the use of student accommodations and modifications as outlined in the students' Individualized Education Plan (IEP). Academic Support: Encourage and support student participation in scheduled lessons throughout the school day (i.e. individualized, small group, whole group, etc.). Social/Emotional Support: monitor/document students' ability to initiate/respond to social interaction from peers; model appropriate social behavior; model emotional regulation strategies (supported by special education teachers and/or Counselor) and monitor students' ability to co-regulate. Behavior Support: Keep the student safe and ensure the student engages in safe behavior with their classmates; Engage students' in work and ensure that the student follows classroom norms and expectations Ability to reteach lessons to students Encourage self-discipline, independence, and responsibility Participate in professional development provided by the school on Montessori pedagogy, the appropriate use of Montessori classroom materials, and approved modifications of these materials. Stay with all assigned students at all times, but fall back when the student is engaging appropriately and independently Collaborate with classroom Guide and Assistant(s) to fully understand classroom routines; assist student in daily routines of the classroom (including greeting the Guide and transitions between activities in the class). Implement any behavior or crisis plans that the student may have, including Behavior Intervention Plans as designed by a BCBA. Maintain data systems for all assigned students; collect and keep data as requested, including frequency data for behaviors, student activities, etc. Engage with all students' related service providers, be present and participate (when appropriate) during the delivery of related services, and provide continuity between classroom interventions and related services. Communicate effectively and professionally with colleagues, specifically the SPED Teacher and Special Education Coordinator Participate in meetings, including: Biweekly meetings of the student support team Weekly meetings of the classroom team IEP meetings (as necessary) Inter-departmental meetings (as necessary) All staff meetings Assist daily with arrivals, dismissals, and before care Assist with classroom set up following dismissal Participate in other school functions as requested by the Executive Director Other duties, as assigned Required Competencies and Qualifications: Have a firm commitment to upholding the school's mission and anti-bias/anti-racist pedagogy. A Dedicated Aide at Capital City: Supports the delivery of effective differentiated instruction, under the guidance of classroom and special education teachers, by: Engaging with, or closely supervising the assigned student at all times. Modeling appropriate spoken and written language skills at every opportunity. Coaching assigned student in Core Values, social problem-solving, Working with the student on academic tasks specified in the daily lesson plan. Collecting observations and data on student learning and behavior. Supports positive student behavior and classroom and school culture by: Modeling and encouraging the student and classmates to succeed in the Core Values (Courage, Compassion, Contribution, Integrity, and Self-Discipline). Implementing supports and consequences specified in the student's Behavior Intervention Plan (as needed). Responding to student behavior using Redirection, Reflection, and Conferencing (Responsive Classroom/Developmental Designs), when a different response is not specified in the behavior plan. Reviewing classroom lesson plans, anticipating challenges, and participating in generating possible solutions. Maintains and completes documentation of student work and behavior. Supports children's safety and physical well-being during the school day by: Ensuring that an adult is with the assigned child during all hours specified on the IEP, by being on time to pick up the assigned child from related services, and by moving with the assigned child to all locations, where the child isn't working 1:1 with another adult. Accompanying assigned child within the total school environment to ensure safe ratios and continue to provide services as appropriate. Asking for help, when a child's behavior threatens their own or other's safety. Contacts the school counselor during periods of crisis as a means of managing escalating behaviors. Is an active member of the Capital City Staff by: Participating in regular team meetings with the classroom teacher and special education teacher to discuss and maintain a productive working relationship. Taking on other responsibilities as assigned. Desired Qualifications: Candidate must have, at minimum, an Associates degree or a passing score on the Paraprofessional Certification and experience working with students with disabilities. Candidates with experience working with students with autism preferred. Spanish speaking candidates preferred.
