Job Description:
The Advanced Practitioner works in collaboration with physicians to assess, plan, and provide care in an Urgent Care setting.
Examines and treats general as well as urgent care patients to ensure proper injury care, disease prevention, diagnosis, treatment, and recovery.
Prescribes and administers medication, performs routine vaccinations, and provides advice as needed.
Requests necessary tests and follow-up visits and refers patients to specialists as necessary.

Duties & Responsibilities

Primarily responsible for conducting subbing sessions for other clients to sustain the productivity margin if a session is cancelled by the family. Subbing sessions will fall on Saturdays and within the employee's availability.
Additional hours or sessions may be assigned by the Regional leadership within employee's availability.
Implements individualized treatment goals and Behavioral Intervention Plans in response to the direction provided by the Clinical Director and Senior Clinical Leader/Clinical Leader and/or other clinical staff in order to improve the behavior and skill acquisition of clients diagnosed with Autism Spectrum Disorder.
Travels to Center, Home, and other community settings to deliver services.
Subbing sessions will primarily take place in the regional centers

General Purpose The primary purpose of your job position is to provide each of your assigned residents with routine daily nursing care and services in accordance with the resident's assessment and care plan, and as may be directed by your supervisors.
* Use the wristband or photo card file to identify residents before administering treatments, serving meals, etc., as necessary.
* Use only authorized abbreviations established by this facility when recording information.
* Report all changes in the resident's condition to the Nurse Supervisor/Charge Nurse as soon as practical.
* Record all entries on flow sheets, notes, charts, etc., in an informative, descriptive manner.
* Report all accidents and incidents you observe on the shift that they occur.
* Agree not to disclose resident's protected health information and promptly report suspected or known violations of such disclosure to the Administrator.
* Report any known or suspected unauthorized attempt to access facility's information system.
* Perform only those nursing care procedures that you have been trained to do.
* Ensure that the resident's room is ready for receiving the resident (i.e., bed made, name tags up, admission kit available, etc.).

* Observe and report the presence of pressure areas and skin breakdowns to prevent decubitus ulcers (bedsores).
* Report injuries of an unknown source, including skin tears.
* Observe disoriented and comatose residents. Record and report data as instructed
* Perform special treatments as instructed.
* Immediately notify the Nurse Supervisor/Charge Nurse of any resident leaving/missing from the facility.
* Use only the equipment you have been trained to use.
* Report defective equipment to the Nurse Supervisor/Charge Nurse
* Maintain the confidentiality of all resident care information including protected health information. Report known or suspected incidents of unauthorized disclosure of such information.

Qualification
Education and/or Experience Must possess, as a minimum, an 10th grade education
Language Skills Ability to read, analyze, and interpret general business periodicals, professional journals, technical procedures, or governmental regulations. Ability to write reports, business correspondence, and
procedure manuals. Ability to effectively present information and respond to questions from managers and employees.
Mathematical Skills Ability to apply concepts such as fractions, percentages, ratios and proportions to practical situations.
Reasoning Ability Ability to solve practical problems and deal with a variety of concrete variables in situations where only limited standardization exists. Ability to interpret a variety of instructions furnished in written, oral, diagram, or schedule form.
Certificates, Licenses, Registrations Must be a licensed Certified Nursing Assistant in accordance with laws of the state.
