Utilizes proper positioning techniques when re-positioning patient in bed, with ambulation or while transferring in & out of the bed or chair.
During provision of care, maintains a safe and secure environment during provision of care.
Follows patients plan of care and accurately documents care provided and any other pertinent information concerning the patient &/or family. Documentation is completed timely as defined by agency

REPORTS TO: Charge Nurse
PURPOSE OF YOUR
The primary purpose of your position is to assist residents in activities of daily living that are outlined in this , and may be directed by your supervisors, i.e., Nurse Team Leader, Neighborhood Charge Nurse, Neighborhood Coordinator or Certified Nurse Aide.
JOB FUNCTIONS

MAJOR DUTIES AND RESPONSIBILITIES
Report all changes in the residents condition to the Nurse Team Leader/Neighborhood Charge. Nurse as soon as practical.
Report all accidents/incidents you observe on the shift as they occur.
Make resident comfortable.
Report all complaints and grievances made by residents to the Charge Nurse.
Follow established policies concerning exposure to blood/body fluids.
Answer resident call lights promptly.
Ensure that residents who are unable to call for help are checked frequently.
Check each resident routinely to ensure that his/her needs and wishes are being met.
Complete visual checks and document in POC. Document frequent checks on the clipboard.
Immediately notify the Nurse Leader/Neighborhood Nurse of any resident leaving/missing from the facility.
Check oxygen tank levels.
Ensure Residents identity before administering service, serving meals, etc.
Prepare residents for meals; wash hands, position tables, place clothing protectors, take to/from dining room, etc.
Assist with passing fluids in the dining room.
Serve food trays, Assist with cutting foods, providing condiments and assist in dining room supervision.
Assist residents with identifying food arrangements (i.e., informing resident with vision problems of foods that are on his/her tray, where it is located, if it is hot/cold, etc.).
Keep residents water containers filled with fresh water at least every eight hours and within easy reach of the resident. Pass ice waters. Refer to the list of who receives thickened liquids and special glasses.
Serve between meal and bedtime snacks and prepare snacks as assigned.
Perform after meal care; remove trays, clean residents hands, face, and clothing. Bring dirty clothing protectors bag to the linen bucket.
Handle and prepare foods and fluids in accordance with sanitary regulations.
Assisting in cleaning kitchen after each use.
Take and place meal orders.
Transport residents to and from dinning services, activities and any other place they would like to go via wheelchair.
Assist in transporting residents to/from appointments, activities and social programs, etc., as necessary.
Assist residents in preparing for activity and social programs (i.e., church services, parties, visitors, etc.).
Visit with residents who cannot or will not attend activities. Maintain a clean activities space, cupboards, appliances, storage room and activities equipment, maintain pantry, utility rooms, shower rooms, nurses stations, supply room, carts and equipment, as assigned.
Assist life enrichment with activities.
Inventory and mark the residents personal possessions, as instructed.
Assist residents with packing their personal possessions when they are being transferred to a new room or when being discharged.
Apply lotion to residents skin.
Assist Certified Nurse Aide with changing of bed linens. Keep lines tight to avoid wrinkles from forming under the resident.
Put extra covers on bed, as requested.
Watch for and report any change in room temperature, ventilation, lighting, etc.
Disinfect and clean high touch surface areas. Mist hand rails and door knobs.
Complete personal possession sheets.
Assist residents in picking out their clothing.
Organize residents closet and ensure clothing items are labeled properly.
Check rooms for food articles (i.e., food in proper containers).
Keep the nurses call system within easy reach of the resident.
Clean resident rooms/closets/medicine cabinets with permission.
Pass linens and remove excess linens from resident rooms.
Clean the clean linen rooms and soiled utility rooms.
Assist residents with sweaters, jackets and lap robes.
Comb or brush the residents hair.
Greet residents with a smile and respect.
Create and maintain an atmosphere of warmth, personal interest, and positive emphasis as well as a calm environment throughout the neighborhood and shift.
Perform all other duties as may be assigned by your supervisor.
Sanitary Requirements:
Wash hands before and after performing any service for the resident.
Follow established safety precautions in the performance of all duties.
Keep residents personal possessions off the floor and properly stored.
Keep floors dry. Wipe up spills immediately.
Keep excess supplies and equipment off the floor. Store in designated areas.
Wash wheelchairs, walkers, etc., as instructed.
Clean, disinfect, and return all resident care equipment to its designated storage area after each use.
Perform routine housekeeping duties (i.e., clean bedrails, over bed table, night stand, etc. that relate to nursing care procedures.)
Report all hazardous conditions and equipment to the Nurse Team Leader/Neighborhood Charge Nurse immediately.
Report all safety violations.
Follow established procedures in the use and disposal of personal protective equipment.
Equipment and Supply Function:
Use only the equipment you have been trained to use.
Operate all equipment in a safe manner.
Use only the equipment and supplies necessary to do the job. Do not be wasteful.
Report defective equipment to the Nurse Leader/Neighborhood Nurse.
Inform the Nurse Leader/Neighborhood Nurse of your equipment and supply needs.
RESIDENT RIGHTS:
Maintain the confidentiality of all resident care information.
Ensure that you treat all residents fairly and with kindness, dignity and respect.
Ensure that all nursing care is provided in privacy.
Knock before entering the residents room.
Report all grievances and complaints made by the resident to the Charge Nurse.
Report all allegations of resident abuse and/or misappropriation of resident Property to Charge Nurse immediately.
WORKING CONDITIONS:
Works throughout the nursing service area (i.e., nurses stations, residents rooms, etc.).
Sits, stands, bends, lifts and moves intermittently during working hours.
Is subject to frequent interruptions.
Is involved with residents, personnel, visitors, government agencies/personnel, etc.; under all circumstances.
Is subject to hostile and emotionally upset residents, family members, personnel, and visitor.
Communicates with nursing personnel and other department personnel.
Is subject to falls, burns from equipment, odors, etc., throughout the workday.
Is subject to exposure to infectious waste, diseases, conditions, etc., including HIV, COVID-19 and Hepatitis B viruses.
