Job Description:
The Advanced Practitioner works in collaboration with physicians to assess, plan, and provide care in an Urgent Care setting.
Examines and treats general as well as urgent care patients to ensure proper injury care, disease prevention, diagnosis, treatment, and recovery.
Prescribes and administers medication, performs routine vaccinations, and provides advice as needed.
Requests necessary tests and follow-up visits and refers patients to specialists as necessary.
Seeks the expert opinion or advice of a physician whenever a case falls outside the scope authorized by policies and protocol.
General Accountabilities And Essential Functions:
Performs comprehensive assessments of patients, including physical exams, medical histories, and symptom evaluation within an urgent care setting and triages patients to differentiate between immediate and less urgent care.
Diagnoses and treats a variety of common urgent care conditions such as minor injuries, infections, respiratory ailments, etc.
Consults with physician(s) regarding patient healthcare plans and treatment, as necessary.
Orders, interprets, and evaluates diagnostic tests to identify and assess the patient's clinical problems and healthcare needs.
Performs selected invasive procedures for therapeutic and/or diagnostic reasons within the scope of validated competence.
Initiates and/or recommends medical drug therapies or other forms of related therapeutic treatments and procedures.
Maintains accurate, detailed, and timely documentation of patient care following legal, regulatory, and organizational standards.
Ensures the appropriate use of urgent care resources and makes referrals to specialists when necessary.
Participates in continuing medical education and professional development to stay current with advances in medical knowledge and practice.
Attends required provider and staff meetings.
Accepts and assumes responsibility for special projects as they arise, and all other duties assigned.
Qualification and Licensure Requirements:
Board certification in the respective field (NCCPA for PA, or board certification as a NP)
Current unrestricted Oregon NP or PA state license w/Prescriptive Privileges.
Current certification is dependent upon specific positions to include Basic Life Support (BLS) from the American Heart Association (AHA).
Minimum of 2-years' experience.

About the Opportunity
The primary responsibility of a Behavior Specialist is to implement consistent evidence-based mental health services and Applied Behavioral Analysis programs to clients with Autism Spectrum Disorder and other co-morbid developmental disabilities in response to the recommendations of the Clinical Leader and Clinical Director in the home, center and/or other natural settings whose permanent RBT provider is not available to cover hours or a session.
Duties & Responsibilities
Conducts 100% of scheduled sessions, unless time-off has been granted or the parents of the client have communicated to the clinical team that they would not like for the hours to be provided.
Primarily responsible for conducting subbing sessions for other clients to sustain the productivity margin if a session is cancelled by the family. Subbing sessions will fall on Saturdays and within the employee's availability.
Additional hours or sessions may be assigned by the Regional leadership within employee's availability.
Implements individualized treatment goals and Behavioral Intervention Plans in response to the direction provided by the Clinical Director and Senior Clinical Leader/Clinical Leader and/or other clinical staff in order to improve the behavior and skill acquisition of clients diagnosed with Autism Spectrum Disorder.
Travels to Center, Home, and other community settings to deliver services.
Subbing sessions will primarily take place in the regional centers
Renders completed sessions daily and writes thorough notes
Communicates schedule changes and cancellations to clinical team immediately.
Participates in regular meetings and promptly communicates with Clinical Leaders/Clinical Directors via email and phone, regarding Treatment Plans and Behavioral Intervention Plans, family concerns and questions.
Fulfills the timely completion of necessary clinical documentation and communication of:

Keeps updated with other site-related documents and current clinical reports for each child served.

Follow the policies and procedures of the facility governing the administering of medications to residents. Note and report errors in the administration of medications Assist in developing and implementing procedures or programs of the facility that seek to determine what medication errors are taking place, when, where, and why they occur, and how they can be prevented.
Perform administrative requirements, such as completing necessary forms, charts, reports, etc.

Participate in Quality Assurance programs, the Pharmaceutical Services Committee, and any other facility committee or program that affects the medication pass and seeks to improve the performance and accuracy of the medication process.
Accurately and safely prepare, administer, and document the oral and/or topical medications that are commonly used in this facility and that may be ordered for resident use by the attending physician or the Medical Director.
Verify that any medications brought into the facility by a newly admitted resident are examined and identified by the attending physician or the facility pharmacy or pharmacist Follow facility policies and procedures regarding the destroying of medications; assist in carrying out these rules under the direction of the facility pharmacist or Director of Nursing Services, including the documentation requirements.
Follow the labeling policies and practices of the facility.
Ensure that all medications administered are properly labeled.
Follow facility procedures in regard to charting medications.
Assist in documenting and removing medications that are discontinued by the attending physician.
Follow facility procedures governing drug receipt errors, including notifying the delivery source and the Director of Nursing Services.
Ensure that documentation of the drug receipt error includes a written error report, signed by an observer/witness and you.
Follow facility procedures in holding medications for residents who are transferred from the facility orwhen the attending physician discontinues medications Observe and verify that medication is ingested or applied as directed.
Document any instance where prescribed medication is not administered, including reason(s) for refusal.
Promptly notify the Charge Nurse, Nurse Supervisor, or Director of Nursing Services of any errors or reactions to medications by residents.
Verify the identity of the resident before administering the medication treatment.
Accurately measure, record, and report the vital signs of residents.
Follow the facility's procedures allowing residents to self administer his or her medications.
Be familiar with the meaning and usage of common medical abbreviations, symbols, and terms relative to the administration of medications as used by the facility, pharmacists, and physicians.
Attend and participate in continuing education programs designed to keep you abreast of changes in your profession, as well as to maintain your certification on a current status Agree not to disclose resident's protected health information and promptly report suspected or known violations of such disclosure to the Administrator.
Report any known or suspected unauthorized attempt to access facility's information system.
