Duties
Responsible for assisting the Day Habilitation Team with the planning, organizing, and implementation of the day to day services, including assisting with the implementation of lesson plans, counseling, advocacy, developing community options and acting as a liaison to families and other programs.
Implements the lesson plans and ensures that plans which are to be based on desires and choices of individuals and/or their personal representatives are being implementing consistently and successfully.
Provides daily/monthly service documentation as outlined in the lesson plans and ensures that the documentation is accurate, contemporaneous, timely and billable, per guidelines.
Assist the Day Habilitation Team in supporting individuals in their efforts toward their stated goals.
Works with other staff and volunteers to ensure that individuals have the opportunity to become interactive members of the community.
Assist with the coordination and implementation of individualized support groups who assist individuals in their efforts to meet their stated goals.
Assists QDDP's with filing documents pertaining to participants of the day program as well as the residential program.
Participates as a member of the Residential and Day Habilitation Team.
Keeps abreast of the latest developments in the disabilities field through readings, attendance at seminars/conferences, etc.
Completes all staff trainings in accordance with the requirements of ADMH, and relevant state/federal regulations.
Attends individual and group staff meetings.
Ensures the required intake information/approvals are in individuals' files.
Makes recommendations to the ICF/Residential & Day Services Director and Residential Program Coordinator for equipment, supplies, and staffing needs.
Keeps track of all behavioral related needs and ensures data is collected.
Responsible for assisting the House Managers with residents in the home and during activities, while on duty.
Works as scheduled by Residential Program Coordinator and ICF/Residential & Day Services Director and maintains efficient, quality care.
Assists in implementing active care plans (such as PCPs) for each resident as directed.
Completes documentation for CFAs, PCPs or BSPs, and assists in preparing reports, as required by licensing and certifying agencies and ROI policy; and keeps the House Managers, Residential Program Coordinator, ICF/Residential & Day Services Director and QDDP informed of any problems.
Demonstrates and/or assists residents in daily self-care practices such as personal hygiene, dressing, cooking, home maintenance, shopping, laundry scheduling, and clothes repair to aid independence and foster self-esteem of residents.
Converses with residents to reinforce positive behaviors and to promote social interaction.
Assist the House Managers with the supervision of preparation, cooking and serving of quality, nutritional, family-style meals according to pre-approved menus and recipes and maintains inventory of food and supplies.
Assist the House Managers with the coordination and planning of outside community activities to include the coordination of transportation (van service) to outside community activities, via the ICF/Residential & Day Services Director.
Assists the House Managers with coordination and communication to the Residential Health Services Nursing staff to coordinate transportation (van service) to medical appointments.
Responsible for transporting residents to all scheduled activities and appointments as necessary and instructed by the Residential Program Coordinator and ICF/Residential & Day Services Director.
Must always obey traffic laws and maintain a clean driving record.
Must be able to assist residents on and off vans, as needed.
Helps to maintain cleanliness and safety of the home to provide pleasant living conditions and to meet requirements of the licensing and certifying agencies and ROI policies.

Purpose
To perform non-professional direct patient care duties under the supervision of nursing personnel and to assist in maintaining a positive physical, social, emotional and spiritual environment for the residents.
Essential Functions Job Functions
Assist residents with daily bath and/or shower which may occasionally require the ability to enter a shower space with the resident. Assistance will also include dressing, grooming, dental care, bowel and bladder functions, preparation for activities within the community, reminders of medical appointments, and basic room care.
Provide functions as directed by supervisor including daily and ongoing records including vital signs, weight and measure residents, and observe and report presence of skin breakdowns; review care plans daily; report changes in resident conditions to supervisor; and, record all necessary charting entries and report all accidents and incidents.
Prepare residents for meals and snacks and perform after-meal resident care.
Maintain resident confidentiality; treat residents with kindness, dignity and respect; know and comply with Residents' Rights rules; and promptly report all resident complaints, accidents and incidents to supervisor.
Assist as directed in proper admission, transfer and discharge of residents, inventory resident possessions and report defective equipment to administration.
Provide table service to residents during applicable meal periods.
Provide daily light housekeeping services, such as emptying trash, dusting, bed making, and other items as required in the resident apartments
Understand and use Standards Precautions and follow established infections control, hazardous communication and other safety rules; ensure cleanliness of assigned residents' rooms; properly maintain and record resident restraints; and, promptly report all violations of safety and sanitation rules to supervisor.
Attend and participate in orientation, state required training, educational activities and staff meeting.

Qualifications

Must be 18 years of age or older.

Completion of training program for certification (CNA preferred not required).
Previous experience with geriatric nursing preferred.

CPR certification.
Basic understanding of aseptic and sterile techniques.
