The Behavioral Health Coordinator works with the Regional Manager, Site Administrator, Medical Director, and Psychiatrist to coordinate total contract services. MENTAL HEALTH COORDINATOR ESSENTIAL FUNCTIONS Acts as point of contact for administrative staff for discussion of treatment programs and problem resolution as needed. Coordinates development and revision of policies and procedures for the identification, assessment, and provision of required clinical services for inmates. Supervises the planning, development and implementation of treatment programs contracted to VitalCore. Provides clinical and/or administrative supervision to staff of Behavioral Health Services. Monitors efforts of Behavioral Health Services to ensure clinical services are being provided in a timely, professional manner. Provides and assigns clinical on-call duties. Ensures treatment programs contracted are integrated and appropriately staffed. Provides administrative support to psychiatric staff. Coordinates the facility's Behavioral Health Quality Improvement Program. Attends training and meetings as required. MENTAL HEALTH COORDINATOR REQUIRED EDUCATION, CERTIFICATIONS, REGISTRATIONS AND EXPERIENCE Doctorate or Master's degree in Psychology or related field from an accredited college or university. Care includes physical, psychological, spiritual, and financial comfort. • Participate in Interdisciplinary Team meetings and up-dates the Plan of Care (POC) for assigned patients based on identified problems and needs • Ensure patient care is delivered at the appropriate level of care • With coordination of PCP and medical director, provide management of medication profile to prevent polypharmacy and ensure optimal symptom management • Manage medication profile to ensure hospice is providing medications related to disease process and symptom management • Ensure all services provided are in accordance with the patient's Plan of Care • Ensure the POC involves the active participation of the patient, family, Primary Care Physician, and the Interdisciplinary Team • Provide team members with appropriate information to coordinate all services related to hospice care for assigned patients. Coordination of services include communicating and making assignments for visiting nurses, home health aide/homemaker, chaplain, social worker, volunteer, and ancillary services, as well as, ordering medications and Durable Medical Equipment, and supplies • Provide consultation and supervision to all CNAs and LPNs involved in the care of the patient • Participate in the performance evaluation of clinical members of LPN(s) and CNA(s) involved with the care of case load. • Assist with the orientation of clinical members participating in the care of the assigned caseload • Ensure cost effective practices by self and others • Assist with QAPI (Quality Assurance and Performance Improvements) activities • Work as a team member at all times and uphold the Hospice Code of Ethics • Provide care to manage patient • Utilize the medication formulary whenever possible for the management of patient symptoms • Triage scheduling and symptom management calls from members of IDT • Reports to the Clinical Director as patient condition changes or change in the patient condition that require change in level of care Work Environment/Physical Requirements: The responsibilities of this position involve significant physical activities including standing, lifting, bending, stooping, pushing, pulling and twisting. The employee must be a licensed driver with an automobile that is in good working order and insured in accordance with the organization requirements. The employee may experience a change in work conditions related to weather issues, traveling in rain, snow, etc., and must be able to adapt to the work environment to meet patient care needs. Required Education and Experience: • Registered Nurse graduated from an accredited program currently licensed and in good standing with the Wisconsin State Board of Nursing. This includes but is not limited to outreach via phone, in-person and home visits, according to protocol. -Inform, educate, and prepare victims regarding offender's status - in-custody, out-of-custody, bail, warrant status, etc. Register victims for VINE (Victim Information Notification Everyday) if offender is in custody and/or connect them with Probation & Parole, if necessary. Work with clients to develop a personal safety plan while the offender is incarcerated or out-of-custody. -Assess client's emergency needs and help them access resources available through Sojourner Family Peace Center, the law enforcement system, the court systems (criminal and civil), the DOC system and within the community. -Complete intakes with clients to help identify individual needs and assist with the development of a personal action plan built on formal/informal supports and community resources outlining goals and expectations. Utilize independent critical skills to identify additional challenges such as AODA and mental health issues that can present at varying levels of severity. -Give support as needed for client follow up and ongoing support for cases meeting the criteria for staffing by the High Risk Team. -Must possess basic knowledge of de-escalation techniques and ability to demonstrate when necessary. -Have working knowledge and ability to perform Danger Assessment, LAP and other relevant screening tools. -Know community resources throughout Milwaukee County and maintain active connections to them. -Build and maintain positive communication and professional relationships with law enforcement jurisdictions and external partners. -Assist clients with the process of filing for the different restraining orders (i.e. Domestic Abuse, Harassment, Individual At-Risk and Child Abuse). -Maintain a full understanding of onsite partners including what services are provided and how to make appropriate referrals. Know community resources and maintain active connections to them. -Accompany clients to court, revocation hearings, and other legal proceedings as requested or necessary -Attend case staffing meetings (internal or external) as needed/requested. Position Summary WHAT YOU WILL DO This role will be working closely with Child Protective Services (CPS) Case Managers by providing services to families to reunify children on a permanent basis to families that may have barriers that prevent them from obtaining these services on their own. Examples of the barriers may be mental illness, developmental or physical disabilities, and/or alcohol or drug abuse. SOME ESSENTIAL RESPONSIBILITIES You will be successful in this role by completing the following tasks and responsibilities: * Supervises family visitation and interaction in the community, in homes, and at the agency. * Supports families through teaching of everyday living skills (i.e. nutrition, housekeeping, hygiene, employment support, financial planning); transporting families to various appointments; educating families in parenting skills; and assisting in crisis management. * Ensures information is communicated and understood between families, care providers, and case managers and documents family interactions and visitations in State database. Assesses client's situation, including strengths, challenges, and progress. * Attends court hearings and when necessary, gives testimony to observations. * Participates in childcare for evening hours trauma parenting group, foster care group or other group meeting the agency may hold. * Provides a "drop-in" service to case managers for families that may have partially unsupervised visits. * Assists case managers with administrative tasks, such as record requests and case closures. Salary Information Wage is $23.76 - $25.13/ hour. Full range extends to $31.27/hour.
