Job Description
Summary of position:
Under the general supervision of the KEEP Director of Community Partnerships and Quality Improvement, the position emphasizes optimizing patient's substance use treatment transition from jail to the community, other treatment settings, or other correctional facilities. Both patient facing and administrative, this position will work to ensure the best possible transition of care for patients and programs.
Responsibilities include:
Coordinate and participate in KEEP pre-release planning sessions and initiatives.
Collaborate with medical, nursing, mental health, Reentry and Transition Services, and pharmacy to improve interdisciplinary approaches to transitions of care.
Assist the Director of Community Partnerships (and Quality Improvement) with logistics and documentation for all KEEP's communications with outreach liaisons in community programs, treatment courts, TASC programs, correctional based substance use services, and other treatment settings in which patients may be transferred.
Participate and coordinate continuity of care planning group sessions in all jail-based substance use treatment units/sober living houses.
Conduct patient centered discharge sessions with all patients nearing a known discharge date.
Assist the Director of Community Partnerships (and Quality Improvement) maintain all OTP, correctional, and addiction related regulations (SCOC, OASAS, NCCHC, etc.) are they pertain to discharge planning and community connection.
Review discharge notifications and coordinate pre-release planning sessions in real time.
Communicate with discharged patients, PORT, and other community outreach workers to ensure patient access to prescribed discharge medication and/or access to opioid treatment programs.
With guidance from the Director of Community Partnerships and Quality Improvement, participate in all transitions of care QI initiatives.
See patients in housing areas or discharge trailer (Trailer 8) when necessary.
Verify methadone (or Suboxone) community treatment participation, dosage, and last date of medication for newly admitted patients, as per policy and procedure.
Screen and assess all eligible candidates for the KEEP program and admit them to the program when appropriate.
Complete all required documentation including treatment plans in a timely fashion.
Assist Site Medical Directors (SMD) and physicians in implementing and maintaining the appropriate KEEP Protocol.
Provided patient counseling as needed.
Other duties as assigned by KEEP Medical Director or their designee
Department Preferences:
Bachelor's Degree
Minimum 2 years' experience in the delivery of substance use counseling services and/or working with an incarcerated population
Previous experience working with methadone treatment preferred
Working knowledge of the community, alternatives to incarceration, and harm reduction treatment landscape
Must hold a current NYS Credential Alcohol and Substance Abuse Counselor (CASAC) or CASAC-T or have extensive experience with substance use care coordination
Must be computer proficient and comfortable with an Electronic Medical Record.
Minimum Qualifications
Any individual working in a NYS Office of Addiction Services and Supports (OASAS)-Certified Treatment Program or newly-hired into the System in the title of Addiction Counselor in a NYC Health + Hospitals health care setting must have proof of completion of OASAS “Supporting Recovery with Medications for Addiction Treatment (MAT)” training.
For Assignment to Level II
1. Valid CASAC - Level 1 Certificate issued by OASAS; and one (1) year of full-time, progressively responsible experience performing SUD assessment, treatment planning, individual and group counseling, case and care management and other functions related to SUD prevention and treatment in a SUD treatment program; or
2. Is otherwise considered a QHP, as described in “2” of Qualification Requirements for Level I above, and has one (1) year of full-time, progressively responsible experience performing SUD assessment, treatment planning, individual and group counseling, case and care management and other functions related to SUD prevention and treatment in a SUD treatment program; or
3. Satisfactory equivalent of education, training, and experience. However, all must have a CASAC Level 1 Certificate issued by OASAS or meet the definition of a QHP as described in “2” above.
