The primary responsibilities include providing individual and family counselling in their home, communicating with juveniles and their family court system (as applicable), facilitating treatment team meetings (as applicable), and completing case notes and other documentation in accordance with HIPAA, APA, DCS and COA requirements. *Caseload will be no more than 12 PRIMARY JOB EXPECTATIONS Psychotherapy and Documentation of Treatment Conduct family intake assessments with the Home Based Case Manager in order to develop family treatment plans Provide individual and family therapy and consistently meet productivity and timeliness requirements. Conduct safety assessments with the family, as needed Utilize empirically supported treatment practices and demonstrate fidelity of practice. Screen and monitor clients for suicidal ideation, runaway, self-injurious behavior, as well as other unsafe behaviors and provide follow up recommendations to the treatment team. Adult Intensive Services (AIS) serves clients experiencing Severe and Persistent Mental Illnesses (SPMI) who need a higher level of care than traditional outpatient services may provide. AIS provides responsive, "wrap around" support including individual therapy, case management, psychoeducation, group therapy, and crisis intervention services in virtual, office, and community settings. In addition, AIS provides coordination of care, consultation, and advocacy with internal programs, external agencies, community resources, family members, and other client supports. This is a dynamic and exciting position which requires strong clinical, communication, and problem-solving skills along with a creative, responsive, and trauma informed approach to care. AIS offers many opportunities for growth and learning as well as a flexible schedule that combines time working from home with time working in the office and in the community. This position is based out of Idaho Springs & Evergreen and serves clients living in rural Jefferson, Clear Creek and Gilpin counties. Responsibilities: Conduct telehealth counseling sessions for SUD and/or mental health disorders using PursueCare's platform and EHR system. Collaborate with interdisciplinary team members, operations staff, partnering providers, and referral sources to ensure comprehensive patient care. Maintain accurate and timely documentation in the EHR system, supporting billed CPT codes and treatment outcomes. Assess and intervene in crisis scenarios, advocating for patients and providing necessary resources and referrals. Conduct sessions from a professional, private, and distraction-free environment to uphold patient privacy and comfort. Education & Experience: Master's level degree or above, with a current independent license to practice (LCSW, LCPC, LMFT, PhD, PsyD). Demonstrated proficiency in utilizing technology for telehealth counseling. Prior experience in substance use treatment preferred.
